The specificity of ultrasound-detected bone erosions for rheumatoid arthritis

Abstract: The presence of US-detected erosions is not specific for RA. However, larger erosions in selected joints, especially 2nd and 5rd MCP, 5th MTP joints and distal ulna, were highly specific for and predictive of RA.

“…However, as shown by Zayat, et al, the presence of any erosive changes in the fifth MTP was specific for RA 61 . It should be pointed out that some of the included papers in this review 15,35,39,48,51,53 contain a small number of patients and therefore their importance lies more in supporting evidence from larger studies and demonstrating the development of US in erosion diagnostics and monitoring.…”

“…Discrimination: Responsiveness. Sensitivity to change was assessed in 11 studies and in 2 of them, a reduction in size of erosions in the evaluated joints was reported 55,61 . In 6/11 studies, where sensitivity to change was studied, erosive changes were described as present/absent.…”

“…Zayat et al [28] searched for erosions in multiple sites of the hands and feet of patients with RA, psoriatic arthritis (PsA), gout and healthy individuals, at the level of the foot focusing on 1 st and 5 th MTP. They found that erosions in RA patients are more frequent comparing with other groups but are not specific for RA [sensitivity (Se) 91.4%, specificity (Sp) 32.9%].…”

“…This fact may suggest that the studied patients were observed at an early stage of the disease, before the above-mentioned changes could appear [12,25]. In our research, erosions most frequently appeared in MTP II, but the majority of reports demonstrate the involvement of MTP V and IP in early RA [12,15,31].…”

“…Erosions on joint surfaces -in accordance with the definition proposed by OMERACT, discontinuity of the cortical bone surface, that was visualized in at least two perpendicular planes [15,16]. The presence of erosions was assessed on a 2-grade scale: 0 -lack of erosions, 1 -erosions present.…”
