The significance of blue color in dermatoscopy

Abstract: Among malignant lesions with blue color, structureless peripheral or patchy blue color is a clue for melanoma, while blue clods point to basal cell carcinoma. Pitfalls include seborrheic keratoses, which may show blue color, as well as some nevi, especially combined nevi.

“…In addition, these vessel-like formations can be induced by tumor cell vasculogenic mimicry, a phenomenon observed in a case, there were highly suspicious signs of malignancy such as irregular vessels and a blue-white veil. The prominent structureless bluish color with scattered clods was also highly suspicious of melanoma [11]. The melanoma in our patient grew adjacent to an intradermal nevus, and there have been some reports of intradermal melanoma associated with an intradermal nevus [12].…”

“…When presented in combined nevi, blue coloration is usually located in the center of the lesion. Peripheral distribution can also be observed, like in our case, although it is more commonly exhibited in melanoma (7,8). Since the lesion of our patient presented in the skin area not suitable for daily skin inspection, the patient could not provide reliable information on the possible changes in the size, shape and color of the nevus, indicating the need for surgical excision and histopathological examination.…”

“…A blue color under dermatoscopy is frequently associated with malignancy [ 12 ] . As previously reported [ 1 ] , mucosal melanoma rarely has the dermatoscopic features of cutaneous melanoma, such as a pigment network or streaks, but often shows a blue-whitish veil and atypical vessels.…”
