The Serological Profile of the Autoimmune Hepatitis/Primary Biliary Cirrhosis Overlap Syndrome

Abstract: Concomitant AMA/anti-dsDNA seropositivity can be considered the serological profile of AIH/PBC OS. The combination of UDCA and steroids is effective in achieving persistent biochemical amelioration in most AIH/PBC OS patients.

“…In a recent study, anti-dsDNA antibodies were significantly more frequent in 15 PBC-AIH overlap cases (60%) than in 120 PBC (4%)-and 120 AIH (26%) patients (<0.0001 and 0.01, respectively) [109]. The concomitant presence of AMA and anti-dsDNA was highly specific (98%) for the overlap condition, being observed in 47% of overlaps as compared to only 2% of controls.…”

“…In addition, Zhao et al reported that 41.5% patients with PBC -AIH overlap showed isolated IgM elevation 9) . These findings were not reported from Western countries 2,3,8) , but we believe in the importance in studying elevation of IgM and its relationship with the onset of PBC -AIH overlap.…”

“…Elevated IgM is thus a common finding, as in PBC 1,2,3,5) . However, it is unusual to see an increase in IgM as high as the level of 4,250 mg/dl seen in this case or higher than the IgG level.…”

“…Furthermore, antidsDNA antibodies, which are known to be strongly associated with SLE, were detected in 60% [50] or 56% [51] of patients with AIH-PBC overlap syndrome.…”
