The sarcoidosis-lymphoma syndrome

Abstract: Analysis of 17 cases of coexistent sarcoidosis and malignant lymphoproliferative disease, supplemented with 29 similar cases reported in the literature indicates that this association is not fortuitous. In addition, significantly more malignancies other than lymphoma were found in this group of patients. A sarcoidosis-lymphoma syndrome appears to exist in which malignant lympho-proliferative disease develops at least 5.5 times more often than expected in middle-aged patients with chronic active sarcoidosis, po… Show more

“…Indeed, the existence of a 'sarcoid-lymphoma' syndrome has been proposed which is characterised by onset of lymphoma after the development of sarcoidosis. 5 In general, these features hold true for the reported associations between plasma cell dyscrasias and sarcoidosis. 8 The pathogenesis of DM or SLRs associated with malignancy is not known, but presumably involves a host immune response to tumour antigens.…”

“…Although individual associations between lymphoproliferative disorders (LPDs) and both DM 4 or SLRs 5 have been reported previously, to our knowledge there are no descriptions of this triad. DM is associated with a five-to seven-fold increase in the incidence of malignancy 4 includ-ing in some, but not all studies, associations with LPDs.…”

“…T cells. In addition, this chronic stimulation may result in prolonged B-cell and plasma cell halflives that may increase the risk of genetic mutations, leading to the formation of a malignancy, such as multiple myeloma [18].…”

“…Second, Brincker [7] reviewed 46 cases of lymphoma occurring in patients with sarcoidosis. He reported only 2 cases in which lymphoma preceded sarcoidosis with an additional 2 cases in a subsequent study [9].…”

“…Since then, additional descriptions of this possible connection have been published but without providing a better understanding of this relationship [1]. The sarcoidosis-lymphoma syndrome descried by Brincker [7] [8] was established on the basis of 2 different studies. In a Danish registry of patients with respiratory sarcoidosis, lymphoma occurred 11.5 times more frequently than expected in the general population [8].…”
