The safety of isotretinoin in patients with lupus nephritis: a comprehensive review

Abstract: Oral isotretinoin (13-cis-retinoinc acid) is a derivative of vitamin A and belongs to the first generation of retinoids, which act as synthetic isomers of retinoic acid (RA). It is a very effective agent in a treatment of acne vulgaris; however, multiple side effects related to therapy with retinoids preclude the use of isotretinoin in less severe acne vulgaris. A significant limitation for the administration of isotretinoin appears in case of concomitant kidney disease with a special attention regarding the s… Show more

“…Severe rhabdomyolysis with mortal outcome was reported with using the drug (8). However, there are some case reports which reported that isotretinoin could induce renal damage, some authors suggested that isotretinoin use is safe in renal transplant and haemodialysis patients (14)(15)(16)(17). To our recognition, our study is the first study that investigated the renal functions while using isotretinoin treatment for acne vulgaris.…”

“…Some authors reported that it may be toxic (9)(10)(11)(12)(13). Some authors suggested that it is safe on kidneys (14)(15)(16)(17). Kaya Aksoy et al (11) had been reported that isotretinoin may cause drug-related acute tubulointerstitial nephritis.…”

“…The most active metabolite of VA, all- trans retinoic acid, could shape the pathogenesis of different autoimmune disorders ( 12 ), including SLE ( 13 , 14 ). We and others have shown the protective effects of retinoid treatment on inflammation of the kidney, or lupus nephritis ( 13 15 ), which is one of the most common and life-threatening manifestations of SLE ( 16 , 17 ).…”
