The safety of asthma medications during pregnancy and lactation: Clinical management and research priorities

Abstract: In asthma, a significant portion of the interaction between genetics and environment occurs through microbiota. The proposed mechanisms behind this interaction are complex and at times contradictory. This review covers recent developments in our understanding of this interaction: the ''microbial hypothesis'' and the ''farm effect''; the role of endotoxin and genetic variation in pattern recognition systems; the interaction with allergen exposure; the additional involvement of host gut and airway microbiota; th… Show more

“…Finally, regarding biological therapy, two recent reports have studied the use of omalizumab or anti-Ig-E without showing an increase in the risk of fetal malformations. While we have not found studies in the literature that approve the use of anti-IL5 drugs ( Bonham et al, 2018 ; Chambers et al, 2021 ) (Level of evidence III; 4. Grade of recommendation D).…”

“…Most asthma medications during pregnancy are regarded as well tolerated, especially when considering the maternal and fetal risks of uncontrolled asthma (Table 3). However, given the challenges of studying medication use in the pregnant population, including exclusion from clinical trials, lapses in knowledge remain [47].…”

“…Due to potential risks, oral corticosteroids should be minimized, especially during pregnancy. Close collaboration between the asthma specialist and the obstetrician is essential to develop an appropriate treatment plan that balances asthma control with minimizing potential risks to the developing fetus [ 42 ].…”
