The rotational advancement of medial plantar flap for coverage of foot defect: A case report

Abstract: This case describes the use of the medial plantar artery flap used to cover a lateral foot wound in a 19‐year‐old male with a history of spina bifida. The original operative plan was for coverage with a medial plantar flap based distally on retrograde flow through the lateral plantar artery; however, this had to be revised intraoperatively as his vascular anatomy was not adequate to support a flap of this type. Thus, advancement with rotation modification of the conventional medial plantar flap was performed w… Show more

“…7 Beidas et al described a case report in which the posterior tibial and lateral plantar arteries were too small to elevate a retrograde-flow medial plantar flap. 18 Based on the results of cadaver dissection in the present report, there was no specimen in whom the lateral plantar artery was very small; however, pre-operative imaging methods such as color Doppler sonography might be useful to detect cases unsuitable for this novel surgical procedure.…”

“…It is difficult for the pedicled flaps to reach this area. Beidas used a rotational medial plantar flap to cover a small size defect in this area . However, since the donor site of the medial plantar flap was often included in the injury zone, a free flap was usually selected for reconstruction.…”

“…However, since the size of the medial plantar flap is small, this flap was only chosen for reconstruction of small size defects of the heel. 1,12,27 If the entire calcaneus tubercle was exposed, the pedicled flaps may have size limited. A free anterolateral thigh perforator flap was usually used in our series.…”

“…As described by Martin et al (1991) in their paper, by tailoring in a "Y to V" fashion, the MP flap pedicle, a considerable gain in the arc of rotation can be obtained permitting coverage of more distant defects of the plantar foot. One of the main disadvantages of this flap, as in other reverse-flow variants of the MP flap (Oberlin, Accioli de Vasconcellos, & Touam, 2000), is an unreliable venous drainage (Acikel, Celikoz, Yuksel, & Ergun, 2003;Akita et al, 2014;Beidas, Tan, & Petersen, 2012;Masuoka & Kato, 2004;Park, Lee, Lee, & Baek, 2017).…”
