Abstract Objective: To investigate the effect of lifestyle modification on endometrial receptivity of obese women with polycystic ovary syndrome using ultrasonography.Methods: This observational study was conducted at Dr. Cipto Mangunkusumo General Hospital Jakarta from August 2019 to May 2020. A total of 14 subjects were participated in this study. The subjects were advised to get nutrition counseling by nutritionist and then followed up the endometrial profiles for 6 months by ultrasonography.Results: A total 19 subjects were received nutrition counseling by Clinical nutritionist, but then, only 14 subjects were evaluated the endometrial study by Ultrasonography. There were several significant result between before and after treatment some subjects such as calories, body weight, body mass index, waist circumference (p<0.05) and type of the endometrial vascular zone, endometrium volume, and vascular flow index by ultrasonography (p<0.05). In this study, no significant results have been found on the correlation between dietary changes and changes in endometrial receptivity profiles.Conclusion: No significant correlation was observed between changes in anthropometrics and daily calorie intake with changes in endometrial vascular zones.Keywords: endometrial receptivity, obese, polycystic ovary syndrome, ultrasonography. Abstrak Tujuan : Untuk mengetahui pengaruh modifikasi gaya hidup pada reseptivitas endometrium perempuan obesitas dengan sindrom ovarium polikistik menggunakan ultrasonografi.Metode: Penelitian observasional ini dilakukan di Rumah Sakit Umum Cipto Mangunkusumo Jakarta dari Agustus 2019 hingga Mei 2020. Sebanyak 14 subjek berpartisipasi dalam penelitian ini. Semua subyek disarankan untuk mendapatkan konseling gizi oleh ahli gizi dan dilakukan observasi profil endometrium selama 6 bulan dengan ultrasonografi.Hasil: Sebanyak 19 subjek menerima konseling gizi oleh ahli gizi klinis, tetapi kemudian, hanya 14 subjek yang dievaluasi profil endometrium dengan Ultrasonografi. Ada beberapa hasil yang signifikan antara sebelum dan sesudah perawatan beberapa subjek seperti kalori, berat badan, indeks massa tubuh, lingkar pinggang (p<0,05) dan tipe zona vaskular endometrium, volume endometrium, dan indeks aliran vaskular dengan ultrasonografi (p <0,05). Dalam penelitian ini, tidak ditemukan hasil yang signifikan pada korelasi antara perubahan pola makan dan perubahan dalam profil reseptivitas endometrium.Kesimpulan: Tidak ada korelasi yang signifikan antara perubahan antropometrik dan asupan kalori harian dengan perubahan zona vaskular endometrium.Kata kunci: endometrium, obesitas, reseptivitas, sindrom ovarium polikistik, ultrasonografi

Background:In this study, we aimed to investigate the ability of transvaginal Doppler ultrasonography in predicting in vitro fertilization (IVF) outcome in women undergoing this procedure.Materials and Methods:In this prospective observational study, 65 infertile women underwent IVF procedure in Isfahan, during 2013-2014, were studied. The pulsatility index (PI), resistive index (RI) and peak systolic velocity (PSV) of the uterine arteries and arcuate arteries were measured in all selected women using transvaginal color and pulsed Doppler measurements on the day of human chorionic gonadotrophin injection. The women followed up for the primary endpoint which was a successful pregnancy. The mean of PI, RI, and PSV were compared in groups of women who had successful IVF and not. The receiver operating characteristic (ROC) curve was used to determine the predictive value of studied indices.Results:In this study, from 65 women, 32 (49.2%) and 33 (50.8%) have successful and unsuccessful IVF outcome, respectively. The mean of PI and RI of both uterine and arcuate arteries were significantly lower in pregnant women than non-pregnant ones (P < 0.001). The area under the ROC curve of PI (84.7%) and RI (84.4%) for uterine arteries was higher than other indices. The most accurate indices for predicting the outcome of IVF was RI of uterine arteries with an accuracy of 81.5%.Conclusion:The findings of this study indicated that PI and RI assessments of uterine arteries could be used as a routine non-invasive factor, before hCG stimulation, for predicting the outcome of IVF.

Low-level laser therapy (LLLT) has been used in photobiomodulation to promote healing, regenerate, and restore tissue function. Women with persistent thin endometrium were assumed to encounter diminished activity in the regenerative and functional capacity of their endometrium. The aim of this study was to examine the ability of LLLT in 635 nm wavelength to enhance the proliferation and gene expression of in vitro cultured endometrial cells. Single (SE) versus multiple exposures (ME) to LLLT were examined in the study groups and compared to controls. A fluence dose of 4.27 J/cm for 16 min was given once in the SE or divided in three equal sessions in days 2, 4, and 6 of the culture in ME. Cellular response was evaluated by measuring viable cell numbers and surface area. Pattern of MUC1, ITGA5, ITGB3, LIF, and PTEN gene expression was assessed using the qRT-PCR. Greater numbers of cells were found in both study groups (P < 0.001) as compared to controls. The surface area of cells at the end of culture phase was highly significant (P < 0.001) in ME when compared to SE and controls. A statistically significant difference was found in terms of gene expression in both irradiated groups (P < 0.001) as compared to controls, although greater difference in PTEN tumour suppressor gene (Phosphatase and tensin homolog) expression was toward ME. The introduction of LLLT to the armamentarium of infertility will serve as a new adjuvant therapy in this field. The current study proofed that LLLT was able to increase the proliferative and functional capacity of cultured endometrial cells.
