Background 131 I treatment is one of the important methods of comprehensive postoperative treatment for patients with hyperthyroidism complicated with differentiated thyroid cancer (DTC). Early identification of patients with poor treatment efficacy of 131 I is particularly important. Current studies mainly focus on the relationship between hyperthyroidism and the occurrence and development of DTC, and there are few studies on the factors affecting the curative effect. The purpose of this study was to find the influencing factors of efficacy evaluation and provide evidence for early identification of patients with poor efficacy in DTC combined with primary hyperthyroidism patients. Methods This was a retrospective analysis of DTC patients with primary hyperthyroidism who received 131 I treatment in our department from 2012 to 2021. Follow-up intervals were 3 months within 1 year, 6 months within 1 to 2 years, and annual follow-up thereafter, the median follow-up time was 12.0 (3.0, 24.0) months. Serological examination and imaging examination were used to evaluate the efficacy. Patients were classified into an excellent response (ER) group and a non-ER group based on treatment response more than 6 months after 131 I treatment. Univariate analysis and multivariate logistic regression analysis were performed on the basic clinical characteristics, pathological characteristics and curative effect of the patients, in order to find independent risk factors affecting the curative effect. Results Eighty-nine patients were mostly female (80.9%), the average age was 43.47±11.88 years old, and tumor size was 1.2 (0.75, 1.80) cm, 56 patients (62.9%) in the ER group. psTg [odds ratio (OR): 1.325; 95% confidence interval (CI): 1.135–1.547; P<0.001], maximum tumor diameter (OR: 2.428; 95% CI: 1.392–4.235; P=0.002) and pathology-confirmed combined HT (OR: 8.669; 95% CI: 1.877–40.038; P=0.006) were independent risk factors for predicting ER. Conclusions Our findings demonstrate that most hyperthyroidism combined with DTC patients could get favorable clinical outcomes from 131 I treatment. The tumor diameter, pathology-confirmed diagnosis of combined HT, and psTg level can be used to identify patients who can get ER by the effect of 131 I in hyperthyroidism combined with DTC at an early stage.

Thermal ablation for cervical lymph node metastasis from papillary thyroid carcinoma: A meta-analysis

Background: Traditionally, surgery has been the standard treatment for cervical lymph node metastasis in patients with papillary thyroid carcinoma (PTC). However, thermal ablation is currently recommended by several guidelines. This study aimed to evaluate the efficacy and safety of thermal ablation for lymph node metastasis in patients with PTC. Methods: We searched PubMed, Embase, Web of Science, and China National Knowledge Infrastructure databases until March 2022 to collect studies on thermal ablation (including radiofrequency, microwave, and laser ablations) for cervical lymph node metastasis from PTC. Results: A total of 190 patients were included, ranging from 5 to 39 in each study, with a sex ratio (male/female) ranging from 1/4 to 17/20, an average age ranging from 15.6 ± 3.0 to 62.3 ± 13.2 (yr), and a total of 270 cervical lymph nodes, ranging from 8 to 98. The follow-up results showed that thermal ablation significantly reduced the maximum diameter and volume of metastatic lymph nodes in PTC ( P < .01). The pooled complete disappearance rate was 86% (95% confidence interval 79% to 93%). Thyroglobulin levels were significantly lower after surgery ( P < .01). No major complications occurred, and the combined voice change rate was as low as 1% [CI 0% to 3%]. Conclusion: Our meta-analysis showed that thermal ablation is an effective and safe method for the treatment of cervical lymph node metastases from PTC. Considering the limitations of this study, more prospective, multicenter, large-sample studies are needed in the future.

In this work, we propose a novel immunoassay platform for the detection of human Thyroglobulin (Tg) to be integrated with fine-needle aspiration biopsy for early identification of lymph node metastases in thyroid cancer patients. The sensing platform detects Tg (a well-known biomarker for the classification of metastatic lymph nodes related to thyroid cancer) by a sandwich immunoassay involving a self-assembled surface-enhanced Raman scattering (SERS) substrate assisted and empowered by functionalized gold nanoparticles enabling additional Raman signal amplification and improved molecular specificity. The sandwich assay platform was preliminary validated in a planar configuration and a detection limit as low as 7 pg/mL was successfully achieved. The sandwich assay was successfully demonstrated on washout fluids of fine needle aspiration biopsies from cancer patients and confirmed the high specificity of the proposed methodology when complex biological matrices are considered. Finally, optical fiber SERS optrodes were fabricated and successfully used to detect Tg concentration by applying the same bio-recognition strategy. This opens the possibility of transferring the Tg detection approach to the optical fiber tip to develop point-of-care platforms that can be directly integrated into fine needle aspiration biopsies.
