Neutrophil to lymphocyte ratio (NLR) is a simple, noninvasive, inexpensive inflammatory marker that can useful in the assessment of inflammatory activity, especially in pediatric ages. The aim of our study was to establish correlations between the presence of Helicobacter pylori (HP) proved histologically and NLR in children. A prospective, case–control study was performed on 137 pediatric patients aged between 1 and 18 years, admitted in a Pediatric Tertiary Hospital from Romania, between April 2016 and January 2018. According to the histologic examination, the children were divided into 2 groups: group 1: 50 children with HP infection, and group 2: 87 children without any pathologic findings. The mean age for the study group was 12.86 ± 3.796 years, whereas for control group, it was 12.10 ± 3.879 years ( P = .3001). HP infection was significantly more frequent among children from rural area ( P = .0089). Epigastric pain and loss of appetite were significantly associated with HP infection ( P = .0350 / P = .0281). We noticed that the leukocyte and neutrophil counts were significantly higher in group 1 ( P = .0076/ P = .0306). We did not find any significant statistical differences between the 2 groups in terms of lymphocytes, erythrocyte sedimentation rate, and NLR or other assessed laboratory parameters. Regarding the IgA antibodies anti-HP and rapid urease test, they were both significantly associated with histologically confirmed HP infection ( P < .0001). Even though, we did not identify significant differences in term of NLR between HP-induced gastritis children and healthy controls, the mean NLR values were higher HP-positive patients.

Can Simple Tests Prior to Endoscopy Predict the OLGA Stage of Gastritis?

Gastritis is a progressive disease that evolves from a non-atrophic to atrophic state and progresses through intestinal metaplasia, with some cases leading eventually to gastric cancer. Since gastritis by definition is an inflammatory process of the mucosal lining of the stomach and is usually associated with pain, we aimed to identify any association between the severity of gastritis and pain and a simple inflammatory marker derived from a complete blood count (CBC). This was a prospective cross-sectional study which enrolled 155 consecutive adult patients who underwent an upper endoscopy. Prior to the endoscopy, all patients were given a questionnaire, numerical rating scale (NRS) and complete blood count evaluation. The biopsy was obtained from the gastric mucosa according to the modified Sydney classification and scored with the Operative Link for Gastritis Assessment (OLGA) scoring system. The results showed a significant correlation between NRS and intestinal metaplasia (p < 0.01); moreover, a correlation was also found between the NRS and OLGA stage (r = 0.469, p < 0.001). A nonlinear curve was constructed for OLGA stage estimation according to NRS scores (r2 was found to be 0.442, with p < 0.001). The results also showed a correlation between the neutrophil to the lymphocyte ratio (NLR) and OLGA stage (p < 0.01). No correlation was found between the other gastric parameters and NLR (p > 0.05). Helicobacter pylori positivity did not correlate with NRS and NLR. As a conclusion, pain measured by NRS and NLR, which are simply calculated from the CBC prior to endoscopy, may be used to predict OLGA stages and estimate the severity of gastritis in endoscopy patients.

Objectives: Helicobacter pylori infection is common all over the world. The present study aims to have platelet indices; to examine the role of platelet count, mean platelet volume (MPV) and platelet distribution width (PDW) as useful, inexpensive, non-invasive biomarkers for monitoring helicobacter pylori therapy. Methods: A total of 72 patients who were determined using helicobacter pylori positivity, gastroscopic biopsy and administered eradication treatment were included in this study. Whether these patients were eradicated after treatment was evaluated by testing helicobacter pylori antigen in stool. Eradication was observed in 48 of the patients, and the demographic data and laboratory parameters of the patient group before and after treatment were compared. Results: The mean age of the patients included in this study was 41.1±12.1. Of the patients, 54 were female and 18 were male. After Helicobacter pylori eradication, a decrease in platelet count and an increase in PDW were detected. The difference was statistically significant (p=0.045 and 0.015, respectively). We did not find any significant difference concerning platelet lymphocyte ratio, neutrophil-lymphocyte ratio and MPV in patients with Helicobacter pylori eradication (p>0.05). ÖZET Amaç: Helikobakter pilori (HP) enfeksiyonu tüm dünyada yaygın olarak görülmektedir. Bu çalışmanın amacı, trombosit indeksleri olan; trombosit sayısı, ortalama trombosit hacmi (MPV) ve trombosit dağılım genişliğinin (PDW) helikobakter pilori tedavisinin izlenmesinde yararlı, ucuz, invaziv olmayan biyobelirteçler olarak rolünü incelemektir. Yöntem: Çalışmaya, HP pozitifliği, gastroskopik biyopsi ile tespit edilmiş ve eradikasyon tedavisi verilmiş toplam 72 hasta dahil edilmiştir. Bu hastaların, tedavi sonrası eradike olup olmadığı, gaitada HP antijen testi yapılarak değerlendirilmiştir. Hastaların 48'inde eradikasyon sağlandığı görülmüş olup, hasta grubunun tedavi öncesi ve sonrası, demografik verileri ve laboratuvar parametreleri karşılaştırılmıştır. Bulgular: Çalışmaya dahil edilen hastaların yaş ortalaması 41.1±12.1'di. Hastaların 54'ü kadın, 18'i erkekti. HP eradikasyonu sonrasında trombosit sayısından azalma ve PDW'de artış saptandı. Aradaki fark istatistiksel olarak anlamlıydı (sırası ile p=0.045 ve 0.015). HP eradikasyonu sağlanan hastalarda trombosit lenfosit oranı, nötrofil lenfosit oranı ve MPV açısından anlamlı bir farka rastlamadık (p>0.05). Sonuç: Çalışmamızda HP tedavisi alan ve eradikasyon sağlanan hastalarda, periferik trombosit sayısında azalma ve PDW de artış saptadık. MPV değerinin ise değişmediğini gördük. Birçok sağlık kuruluşunda çalışılabilen ve ucuz bir tetkik olan, trombosit sayısı ve PDW değerinin, HP eradikasyon tedavi sürecini takip etmek için kullanılabilecek yararlı parametreler olabileceğini düşünüyoruz.
