The role of steroid treatment in intractable cystitis glandularis: A case report and literature review

Abstract: Cystitis glandularis is a proliferative disease of the urinary bladder epithelium. It is rare in children. We report a case of a 23-yearold female with intractable macroscopic hematuria and severe irritative bladder symptoms persisting for 13 years. The patient, who had undergone open and endoscopic bladder surgery at various medical centres, is currently being followed up at our clinic. Cystoscopy revealed multiple edematous papillary tumours on the bladder neck, trigone, and lateral wall on both sides and sh… Show more

“…Whether the proliferation and the chronic obstruction could have been improved with an alternative medical management, for example with high-dose corticosteroids, is uncertain. 16 No spontaneous improvement was observed, although the patient decided to postpone the surgery. Ultimately, the pyeloureteral anastomosis offered a definitive treatment.…”

“…Pathophysiological mechanisms leading to hyperplasia of von Brünn nests have been poorly studied, but are believed to include ischemia, infection, inflammation, chemotherapy, and radiotherapy. 7 , 9 , 10 , 11 , 16 , 17 One study has shown that cells of von Brünn nests synthesize and express fibroblast growth factor (FGF)−10 receptor, and that a paracrine synthesis of FGF-10 is present in the vicinity of von Brünn nests in the exstrophic bladder. 18 FGF-related signaling could therefore be a key factor in the proliferation of von Brünn nests.…”

“…Some researchers believe that nerve-preserving total bladder prostatectomy plus ileal new bladder is also effective [12][13]. In recent years, studies have found that celecoxib, vitamin C Poria cocos polysaccharides and steroids can also be used as one of the treatment strategies [14][15][16][17].…”

“…The treatment of CCG includes transurethral resection and elimination of obstruction or chronic infection. [ 16 ] However, recurrence with resection alone may be common[ 17 ] and various modalities have been described in the literature to deal with the persistent disease: Intravesical Bacillus Calmette–Guérin;[ 18 ] intravesical chemotherapeutic agents;[ 19 ] oral steroids;[ 20 ] intravesical steroids;[ 21 ] laser ablation,[ 22 ] etc. However, optimal dosing schedule is not available for any of them.…”
