The role of psychological factors in response to treatment in neurocardiogenic (vasovagal) syncope

Abstract: This study has confirmed that patients with VVS have a significant degree of psychological distress, which is worthy of consideration in its own right, out with management purely aimed at reducing syncopal or pre-syncopal symptoms. Further, this distress may actually influence the natural history of what is a chronic relapsing condition and may in fact be more relevant to the patient than the number of syncopal episodes that they are experiencing.

“…On the other hand, we and others find that the presence of higher levels of general psychological complaints is associated with an increased risk of syncopal recurrence [8,13]. In our study, the association between general psychological complaints and syncopal recurrence remained significant, also if other potential predictors of syncopal recurrence were taken into account.…”

“…Additionally, in several studies, syncopal recurrence was strongly associated with the number of previous syncopal episodes [2,24]. Syncopal recurrence was also found to be associated with age [7,24] and/or gender [2,24], in some, though not all studies [8,12,13].…”

“…9,11 In previous studies, QoL has been evaluated only once after treatment initiation, in combined groups of patients with various diagnoses and treatments. 10,12,13,27 In contrast, we prospectively evaluated QoL at consecutive time points during follow-up in a more homogeneous group of patients. We found that both the Physical Component Summary of the SF-36 questionnaire and the SDS significantly improved during treatment including physical counterpressure manoeuvres.…”

“…More focused and multidisciplinary approaches may be required among these individuals, 37 which might include psychiatric evaluation and intervention. 3,38 However, further research is warranted to examine in detail why this association between syncope and detrimental effects on occupational safety and employment occurs and to facilitate better understanding of the content and organization of preventive strategies.…”
