Львів, 79005, Україна Na + /K + -АТФаза -електрогенний мембранний ензим, який є мішенню для дії різноманітних фізичних та хімічних факторів середовища, механізми впливу яких на ензим недостатньо вивчені, що й обумовлює актуальність його подальшого дослідження та аналізу кінетичних параметрів.На сьогодні вплив низькоінтенсивного лазерного випромінювання (НІЛВ) на зародкові об'єкти є не до кінця з'ясованим. Найбільш обґрунтованою є гіпотеза про те, що механізм дії НІЛВ може бути опосередкованим структурами цитоплазматичної мембрани за рахунок поглинання квантів фотоакцепторними молекулами, що призводить насамперед до активації біохімічних реакцій в клітині. Під час ферментативних реакцій відбуваються постійні динамічні перебудови комплексу протеїн-ензиму з одного конформаційного стану в інший, які можна виявити експериментально.В експериментах, виконаних на суспензії плазматичних мембран ембріональних клітин в'юна Misgurnus fossilis L., яку було отримано після одноразового опромінення зародків червоним низькоінтенсивним лазером, досліджували вплив температури на активність маркерного ензиму -Na + /K + -АТФази. У статті подані результати оцінювання змін температурної залежності активності ензиму за умов впливу НІЛВ тривалістю експозиції 5 хв та у контролі. Зокрема встановлено, що максимальна активність Na + /K + -АТФази спостерігається за температури середовища інкубації 22-23 °С як за контролю, так і за дії НІЛВ на всіх досліджуваних стадіях розвитку зародків в'юна. Дія НІЛВ супроводжується збільшенням частки впливу фактора температури на активність ензиму і до зниження частки впливу часового фактора розвитку зародків в'юна порівняно з контролем, а також до зміни енергії активації ензиму. Na + /K + -ATPase is an electrogenic membrane enzyme that is targeted by different physical and chemical factors. The mechanisms of their action on this enzyme are not fully elucidated. The determination of these mechanisms is important task which needs further research and analysis of kinetic properties.The influence of low-intensity laser irradiation on embryo objects is currently obscure. The most reasonable hypothesis nowadays is that the mechanism of low-intensity laser irradiation is mediated with cytoplasmic membrane structures by absorbing photons molecules that primarily leads to activation of biochemical reactions in cell. During enzymatic reactions the constant dynamic adjustment of protein-enzyme complex from one conformational state to another is occurring, which can be experimentally detected.In experiments, performed on a suspension of plasma membranes of embryo cells of loach Misgurnus fossilis L., which were irradiated by low-intensity irradiation red He-Ne laser once after fertilization, the influence of temperature on enzyme activity of Na + /K + -ATPase was studied. BACKGROUND: Non-coronary cardiomyopathies, including inflammatory cardiomyopathies, are one of the leading causes of disability and mortality in children and adults. Inflammatory cardiomyopathy in most cases is an outcome of myocarditis as a result of direct agent-dependent myocardial damage or autoimmune reactions in the absence of genetic predisposition. AIM: On the basis of clinical assessment of cardiovascular system changes, to develop a discriminant model to predict the possibility of myocardial damage of inflammatory genesis in acute intestinal infections (AII) in children. MATERIALS AND METHODS: In the Intestinal Infections Department of the Children’s Research and Clinical Center for Infectious Diseases of FMBA of Russia in the period 2020-2022, 33 children aged from 1 month to 13 years with AII of different etiology were consecutively observed, in whom, according to clinical, laboratory and instrumental examination data, for the first time in their lives, changes in cardiovascular system were detected. The sample was formed using an algorithm for diagnosing inflammatory myocardial lesions in points, the sum of which concluded that there were no or presence of such, which allowed us to identify group 1 (n = 24) and group 2 (n = 9), respectively. Etiological diagnosis of acute intestinal infections was performed in all children, clinical and laboratory data, results of a 12-channel electrocardiogram (ECG), transthoracic echocardiography (EchoCG) were evaluated. The differences in the groups were revealed using the Pearson chi-squared test and the Mann–Whitney test, which were considered reliable at p 0.05. The method of discriminant analysis was used. RESULTS: Functional systolic noise and increased cardiospecific enzymes were observed less frequently in group 1 than in group 2 (4.2%; 22.2% and 67%; 100%). Of the pathological ECG changes, violations of the repolarization process were observed (16.7%; 33.3%). No significant changes in the Echo-KG were detected. The viral etiology of acute intestinal infections dominated (79.2%; 88.9%). Changes in the hemogram were characterized by a high frequency of leukocytosis in group 1 (29.2%; 0%) and leukopenia in group 2 (4.2%; 22.2%). In group 1, there was a higher frequency of neutrophilosis (79.2%; 44.4%), rod-shaped shift (62.5%; 22.2%), thrombocytosis (29.2%; 0%), lymphocytopenia (95.8%; 55.6%; p 0.05), monocytosis (8.3%; 0%); increased C-reactive protein (41.7%; 33.3%), ALT (29.2%; 11.1%), urea (29.2%; 11.1%), leukocyturia (16.7%; 0%). CONCLUSIONS: Clinical evaluation of cardiovascular system changes in children with acute intestinal infections using a discriminant model allows to exclude myocardial damage of inflammatory genesis with high probability (95.7%), which is of practical importance.

РезюмеВ статье описан клинический случай атипичного тяжелого течения парвовирусной В19 инфекции, сходный по своим проявлениям с менингококковой инфекцией, при сочетанном инфицировании менингококком, гемофильной палочкой, вирусом гриппа А и другими бактериальными патогенами. У пациента, госпитализированного с острым респираторным заболеванием и токсикодермией, в течение 24 ч появились новые симптомы: головная боль, геморрагическая сыпь, снижение артериального давления до 80/50 мм рт. ст., лихорадка до 39,8°С. На основании симптомов был поставлен предварительный клинический диагноз: «Менингококковая инфекция, генерализованная форма -менингококцемия, тяжелое течение. Инфекционно-токсический шок 2 ст.». При лабораторном исследовании клинических образцов больного бактериологическими, серологическими и молекулярно-генетическими методами в ликворе, крови и мазках из зева и носа рост бактериальной микрофлоры не обнаружен. В мазке из носовой полости был выявлен генетический материал (ДНК и РНК) Neisseria meningitides, Haemophilus influenza и гриппа А/Н3N2. В то же время в ликворе и крови пациента была выделена ДНК парвовируса В19 в высокой вирусной нагрузке: Ct 21,1 и Ct 4,9 соответственно. У пациента были обнаружены не только IgG-, но и IgM-антитела к парвовирусу В19, что является подтверждением острой парвовирусной В19 инфекции. Выраженная аллергическая реакция подтверждалась высоким показателем общего IgE (1068 Ед/мл). Таким образом, мы описали нетипичный случай лабораторно подтвержденной парвовирусной B19 инфекции при наличии бактериально-вирусной смешанной инфекции и аллергической реакции у пациента.
