The association between gastroesophageal reflux disease (GERD) prevalence and its risk factors in an area with low Helicobacter pylori prevalence is important to clarify. We analyzed the prevalence of GERD and risk factors in an area of Indonesia with low prevalence of H. pylori infection. We recruited 104 dyspeptic patients who underwent endoscopy in Surabaya. Patients were diagnosed with GERD based on the Los Angeles classification. We evaluated gastric biopsy specimens and measured serum pepsinogen levels. Interleukin polymorphisms were evaluated by polymerase chain reaction-restriction fragment length polymorphism. Of 104 patients, 56 (53.8%) were endoscopically found to have GERD, with most categorized as grade A; 48 (46.2%) were classified as non-GERD. Higher economic status, smoking, and a history of proton-pump inhibitor use significantly increased the risk of GERD. GERD Questionnaire scores showed a positive correlation with GERD (P < 0.001). An association was found between antral atrophic gastritis and GERD (P = 0.030), and patients with GERD more frequently had severe antral atrophy than nonerosive reflux disease (P = 0.018). We found an association between pepsinogen I/II levels and GERD (P = 0.047), but with low accuracy. IL-1β -511 TT and CT were predominant among the IL-1β -511 genotypes, and IL-8–251 AT and TT were predominant among the IL-8–251 genotypes. In conclusion, we found a high prevalence of GERD in an area with low prevalence of H. pylori infection, which could be associated with acid reflux. Smoking, history of proton-pump inhibitor use, and higher economic group significantly increased the risk of GERD.

Background: Helicobacter pylori is responsible for gastric cancer in approximately tens of millions of patients. Gastric cancer in Sudan represents one of the top causing death among cancers with about 686 cases per year and a 2.7 % mortality rate. IL-1RN VNTR polymorphism has been reported to increase the risk of gastric cancer. Objective: The purpose of this study was to assess the association of the 86 bp VNTR polymorphism of IL- 1RN gene and the susceptibility to H. pylori infection and gastric cancer in the Sudanese population. Materials and methods: Genomic DNA was extracted from 114 subjects. Of whom 60 had gastritis and duodenitis, 26 had a peptic ulcer, 16 had gastric cancer and 12 had normal gastroscopy findings. H. pylori infection was investigated by specific 16S rRNA. And IL-1RN VNTR polymorphism at intron 2 was genotyped using the PCR method and direct sequencing for random samples. Results: The positive H. pylori infection rate among participants was 47.37%. There is a lack of a significant difference in IL- 1RN genotype with H. pylori infection (p-value=1.0000). The IL-1 RN L/L genotype was significantly more frequent in a patient with benign disorders (gastritis or duodenitis or peptic ulcer), Odd=6.000 (95% CI =1.750-20.57, P=0.0056). While the heterozygote genotype 2/L was associated with an increased risk of gastric cancer with OR = 12.83 (95% CI = 1.261-130.6, P=0.0302). Conclusion: Independently carriage of IL-1RN *2 allele was associated with increased risk of gastric cancer in the Sudanese population. Notwithstanding the relatively small sample size of the study population, our findings show that the host genetic can be a useful tool for identifying high-risk individuals among dyspeptic patients; and also underscore the role played by host genetics in gastric carcinogenesis. To the best of our knowledge, this is the first study in Sudan concerning this issue.

Background: Helicobacter pylori (H. pylori) is considered the main cause of gastritis, peptic ulcer and gastric carcinoma in the human populations. H. pylori infection influences the secretion level of several proinflammatory cytokines including IL-1β, which encoded by the IL-1B gene. Objective: The current study aimed to investigate whether IL-1B gene polymorphisms are associated with H. pylori infection among the Jordanian population and responses to triple therapy. Subjects and Methods: The gastroscopic examination was performed on 412 subjects for H. pylori infection diagnosis, 257 subjects were found to be infected by H. Pylori (positive cases), whereas 155 subjects were uninfected (negative controls). The IL-1B gene T-31C and C3954T polymorphisms were genotyped by PCR-RFLP. Results: It was found that the T-31C polymorphism has a significant association with H. pylori infection (P<0.05), and the TT genotype frequency was significantly higher in infected subjects (50.2%) compared to controls (38.7%). On the other hand, no significant association was detected between C3954T SNPs and H. pylori infection among the Jordanian population. In addition, none of the examined polymorphisms were found to influence the responses to triple therapy. Conclusion: The IL-1B gene T-31C SNP might be associated with an enhanced risk of H. pylori infection among the Jordanian population.
