The Role of Hormone Replacement Therapy in the Prevention of Postmenopausal Heart Disease

Abstract: Coronary heart disease is the single leading cause of death in women and a significant cause of disability. Menopause adversely affects several risk factors for coronary heart disease, suggesting that hormones influence the risk of coronary heart disease in postmenopausal women. This article reviews the observational and clinical trial data evaluating the relation between cardiovascular disease and hormone replacement therapy. Biological mechanisms of estrogen and the impact of adding progestins are emphasized… Show more

“…Elevations in Lp(a) have been associated with coronary heart disease and thrombotic stroke (25)(26)(27). The mechanism of this relationship is not known, but it is thought that because apoA and plasminogen are homologous, elevated levels of Lp(a) may interfere with fibronolysis and therefore promote thrombosis (2).…”

“…ome studies have found the use of hormone replacement therapy (HRT) to be associated with a better cardiovascular health profile in healthy postmenopausal women, whereas others have found no benefits (1)(2)(3)(4)(5)(6). Epidemiological research on the potential benefits of HRT use in postmenopausal women with diabetes is also inconclusive (7)(8)(9)(10)(11).…”

“…This would be expected to reduce the risk of thrombotic events, but large studies would be required to confirm any clinically relevant effect. Although case -control analysis indicates that current hormone replacement therapy may increase the risk of thrombosis during the first year of use 32 , numerous studies have consistently shown that hormone replacement therapy reduces cardiovascular events and mortality 33 , and the relationships remain under debate.…”
