The Role of Frontalis Orbicularis Oculi Muscle Flap for Correction of Blepharoptosis With Poor Levator Function

Abstract: On the basis of the close anatomical interdigitation between the longitudinal-oriented frontalis muscle and the horizontal-oriented orbicularis oculi muscle (OOM), frontalis OOM (FOOM) flap was developed to treat blepharoptosis. Retrospective study during an 11-year period, 66 patients with 81 poor levator function ptotic eyelids accepted FOOM flap shortening (65 lids; 80.2%) or double-breasted FOOM flap advancement (16 lids; 19.8%) to correct blepharoptosis. There were 51 (77.3%) patients with unilateral ptos… Show more

“…Nemet 15 did not find any association between Hering's phenomenon and palpebralis function in a study of 35 patients with congenital ptosis. In a study by Lai et al, 16 only 17.6% of patients with unilateral ptosis who had poor palpebralis function exhibited a positive Hering's phenomenon. Up to now, the relationship between palpebralis function and the Hering's phenomenon is not clear.…”

“…Wide dissection of the OOM for harvesting the double-breasted FOOM flap required longer operative time and yielded longer downtime. 13 , 22 The function-preserving FOOM flap addresses lagophthalmos by performing submuscular dissection from the lower part of the orbital OOM, thereby leaving most OOM in situ without disturbing the integrity of the muscle.…”

“…In cases of poor levator muscle function, surgery with contraction of the frontalis is needed directly via the muscle itself or indirectly via an artificial material between the frontalis muscle and the tarsal plate. The frontalis sling technique has been considered the main procedure to treat patients with severe eyelid ptosis for decades [1][2][3][4][5][6]. Blepharoptosis correction under local anesthesia via frontalis transfer (local FT) can be an effective, but aggressive treatment [7].…”
