The role of DNA methylation in ovarian cancer chemoresistance: A narrative review

Abstract: Background and Aims: Ovarian cancer (OC) is the most lethal gynecological cancer. In 2018, it was responsible for over 180,000 deaths worldwide. The high mortality rate is the culmination of a lack of early diagnosis and high rates of chemotherapy resistance, which is synonymous with disease recurrence. Over the last two decades, an increasingly significant role of epigenetic mechanisms, in particular DNA methylation, has emerged. This review will discuss several of the most significant genes whose hypo/hyperme… Show more

“…Targeted methylation of specific genes may represent an alternative strategy to enhance chemotherapy sensitivity in EOC [ 21 ]. Ideally, in the future, the prediction of chemoresistance and subsequent suitability for DNMTi therapy could be informed by sampling OC patient's plasma cell‐free tumor DNA (cfDNA) for specific methylation‐based biomarkers [ 22 ].…”

“…Song and Artibani reviewed the role of DNA methylation in OC- associated chemoresistance and methylation on ABC transporters. It was found to promote chemotherapeutic drug efflux, whereas downregulation in the expression of proapoptotic genes such as RASSF1A, MLH1 and MSH2 due to hypermethylation caused chemoresistance in cancer tissue samples ( Song and Artibani, 2023 ). Another study in 2022 ( Bisht et al, 2022 ) discussed the role of 5-hydroxymethylcytosine (5hmC), a demethylation intermediate, in OC.…”

“…In clinical scenarios, frequent methylation and downregulation of the hMLH1 gene are observed in germ cell tumors after treatment, implying the potential clinical relevance of DNA mismatch repair deficiency in the context of platinum resistance [71].…”
