Plants including Rhizoma polgonati, Smilax china, and Trigonella foenum-graecum contain a lot of diosgenin, a steroidal sapogenin. This bioactive phytochemical has shown high potential and interest in the treatment of various disorders such as cancer, diabetes, arthritis, asthma, and cardiovascular disease, in addition to being an important starting material for the preparation of several steroidal drugs in the pharmaceutical industry. This review aims to provide an overview of the in vitro, in vivo, and clinical studies reporting the diosgenin’s pharmacological effects and to discuss the safety issues. Preclinical studies have shown promising effects on cancer, neuroprotection, atherosclerosis, asthma, bone health, and other pathologies. Clinical investigations have demonstrated diosgenin’s nontoxic nature and promising benefits on cognitive function and menopause. However, further well-designed clinical trials are needed to address the other effects seen in preclinical studies, as well as a better knowledge of the diosgenin’s safety profile.

Phytosterols (PS) are biologically active steroidal compounds obtained from plant foods and cholesterol is found in animals. They have a prominent role in reducing the low-density lipoprotein (LDL) cholesterol levels, thus decreasing the risk of many diseases. PSs also have anti-cancer, antioxidant, antiulcer, immunomodulatory, antibacterial, antifungal effects and modulate inflammation by promoting the wound healing and inhibition of platelet aggregation. The most challenging part concerned about phytosterols was bioavailability. Phytosterol’s absorption and the concentration of circulation over the body were lesser in human intestine compared to cholesterol because of its selectivity and return through intestinal transporters. We searched PubMed, Scopus, Embase, Google scholar and major conference proceedings. Sixteen such therapeutically potent plant steroids were studied in this systematic review to assess the bioavailability issues of phytosterols. Swiss ADME web tool that gives free access to a pool of fast yet robust predictive models for physicochemical properties, pharmacokinetics, drug-likeness and medicinal chemistry friendliness was used for the study.

According to the International Index of Erectile Function-5 score, a single blind study shows that diallyl thiosulfinate with nuciferine and diosgenin as oral tablets are able to improve the control of ejaculation in patients suffering from premature ejaculation, primary or secondary to erectile dysfunction [ 64 ]. These results suggest that the therapy of these natural products is able to treat premature ejaculation associated with metabolic syndrome.…”

Natural Aporphine Alkaloids with Potential to Impact Metabolic Syndrome

The incidence and prevalence of metabolic syndrome has steadily increased worldwide. As a major risk factor for various diseases, metabolic syndrome has come into focus in recent years. Some natural aporphine alkaloids are very promising agents in the prevention and treatment of metabolic syndrome and its components because of their wide variety of biological activities. These natural aporphine alkaloids have protective effects on the different risk factors characterizing metabolic syndrome. In this review, we highlight the activities of bioactive aporphine alkaloids: thaliporphine, boldine, nuciferine, pronuciferine, roemerine, dicentrine, magnoflorine, anonaine, apomorphine, glaucine, predicentrine, isolaureline, xylopine, methylbulbocapnine, and crebanine. We particularly focused on their impact on metabolic syndrome and its components, including insulin resistance and type 2 diabetes mellitus, endothelial dysfunction, hypertension and cardiovascular disease, hyperlipidemia and obesity, non-alcoholic fatty liver disease, hyperuricemia and kidney damage, erectile dysfunction, central nervous system-related disorder, and intestinal microbiota dysbiosis. We also discussed the potential mechanisms of actions by aporphine alkaloids in metabolic syndrome.

Abstract: In conclusion, our study, even if supported by preliminary results, showed how Diallyl Thiosulfinate, Nuciferine and Diosgenin is able to improve the control of ejaculation in patients suffering from PE, primary or secondary to ED without any significant adverse effects.
