The aim of the present study was to evaluate new bone formation around the periosteum within the space formed by a polytetrafluoroethylene tube on the surface of rat calvarial bone. The experimental model consisted of 40 rats divided into eight groups (five rats per group). Each group was divided into the experimental group and the control groups, and it divided on 1, 2, 6 and 8 weeks group respectively. For the experimental groups, a polytetrafluoroethylene tube was placed under a raised periosteal flap, and for the control groups, the periosteum was removed and a teflon tube was placed under the skin on the calvarial bone. All surgical sites were then sutured. We examined new bone formation using micro-computed tomography, histology by staining with hematoxylin and eosin, and immunochemistry by antibodies specific to bone morphogenetic protein-2 (BMP-2) and runt-related gene-2 (Runx2). The experimental and control groups were compared at 1, 2, 6, and 8 weeks after surgery. The histological analysis clearly demonstrated that specimens from the experimental groups showed some bone formation at 2, 6, and 8 weeks after surgery. In contrast, the control groups demonstrated new bone only at 6 and 8 weeks after surgery. New bone was produced only from the calvarial bone side; it was not observed from the periosteum side. Micro-computed tomography scan revealed that bone mineral content (BMC), bone mineral density (BMC), and bone volume/tissue volume ratio (BV/TV) all increased in a time-dependent manner in the experimental groups. In the control groups, the bone volume/tissue volume ratio, bone mineral content, and bone mineral density increased slightly from 6 to 8 weeks after surgery. In all experimental and control groups, BMP-2 positive and Runx2-positive cells appeared on the new bone. In conclusion, these results suggest that the periosteum plays a role in promoting new bone formation.

This study was designed to observe the effects of bone morphogenetic protein-7 (BMP-7) ex vivo gene therapy on callus formation during rat mandibular distraction osteogenesis (DO). Fifty-four Sprague-Dawley rats underwent osteodistraction of the right mandible and were then randomly divided into three groups. Immediately after distraction, autologous bone marrow mesenchymal stem cells (MSCs) transfected with BMP-7, MSCs untransfected with BMP-7, and physiological saline were injected into the distraction gaps of the mandibles in groups A, B, and C, respectively. Nine animals from each group were euthanized at 2 and 6 weeks after completion of distraction. The distracted mandibles were removed and processed for radiographic, histological, immunohistochemical, and scanning electron microscopic examinations as well as Ca/P ratio analysis. Group A animals showed greater bone formation and earlier mineralization in the distracted callus when compared with group B. Similarly increased callus formation was found in group B than group C. Positive immunostaining of BMP-7 was observed in the distracted callus in all groups. However, BMP-7 expression was much stronger in group A compared with groups B and C. The results of this study suggest that BMP-7-mediated ex vivo gene transfer based on MSCs may accelerate callus formation in distraction osteogenesis and facilitate consolidation. Local gene therapy may ultimately be an alternative or supplemental approach to DO enhancement, especially for patients whose osteogenic potentials are compromised by diseases such as osteoporosis, severe trauma, and postoncologic irradiation. ß

This study examines the effect of a hyaluronic acid (HyAc) containing tricalcium phosphate putty scaffold material (TCP-P) and of a particulate tricalcium phosphate (TCP-G) graft on bone formation, volume stability and osteogenic marker expression in biopsies sampled 6 months after bilateral sinus floor augmentation (SFA) in 7 patients applying a split-mouth design. Biopsies were processed for immunohistochemical analysis of resin embedded sections. Sections were stained for collagen type I (Col I), alkaline phosphatase (ALP), osteocalcin (OC) and bone sialoprotein (BSP). Furthermore, the bone area and particle area fraction were determined histomorphometrically. Cone-beam CT data recorded after SFA and 6 month later were used for calculating the graft volume at these two time points. TCP-P displayed more advantageous surgical handling properties and a significantly greater bone area fraction and smaller particle area fraction. This was accompanied by significantly greater expression of Col I and BSP and in osteoblasts and osteoid and a less pronounced reduction in grafting volume with TCP-P. SFA using both types of materials resulted in formation of sufficient bone volume for facilitating stable dental implant placement with all dental implants having been in function without any complications for 6 years. Since TCP-P displayed superior surgical handling properties and greater bone formation than TCP-G, without the Hyac hydrogel matrix having any adverse effect on bone formation or graft volume stability, TCP-P can be regarded as excellent grafting material for SFA in a clinical setting.
