Arginine vasopressin (AVP) plays a major role in the homeostasis of fluid balance, vascular tonus, and the regulation of the endocrine stress response. The measurement of AVP levels is difficult due to its short half-life and laborious method of detection. Copeptin is a more stable peptide derived from the same precursor molecule, is released in an equimolar ratio to AVP, and has a very similar response to osmotic, hemodynamic, and stress-related stimuli. In fact, copeptin has been propagated as surrogate marker to indirectly determine circulating AVP concentrations in various conditions. Here, we present an overview of the current knowledge on AVP and copeptin in perinatology with a particular focus on the baby’s transition from placenta to lung breathing. We performed a systematic review of the literature on fetal stress hormone levels, including norepinephrine, cortisol, AVP, and copeptin, in regard to birth stress. Finally, diagnostic and therapeutic options for copeptin measurement and AVP functions are discussed.

Objective. To investigate the short- and long-term effects of electroacupuncture (EA) compared with medium-frequency electrotherapy (MFE) on chronic discogenic sciatica. Methods. One hundred participants were randomized into two groups to receive EA (n = 50) or MFE (n = 50) for 4 weeks. A 28-week follow-up of the two groups was performed. The primary outcome measure was the average leg pain intensity. The secondary outcome measures were the low back pain intensity, Oswestry Disability Index (ODI), patient global impression (PGI), drug use frequency, and EA acceptance. Results. The mean changes in the average leg pain numerical rating scale (NRS) scores were 2.30 (1.86–2.57) and 1.06 (0.62–1.51) in the EA and MFE groups at week 4, respectively. The difference was significant (P < 0.001). The long-term follow-up resulted in significant differences. The average leg pain NRS scores decreased by 2.12 (1.70–2.53) and 0.36 (−0.05–0.78) from baseline in the EA and MFE groups, respectively, at week 28. However, low back pain intensity and PGI did not differ significantly at week 4. No serious adverse events occurred. Conclusions. EA showed greater short-term and long-term benefits for chronic discogenic sciatica than MFE, and the effect of EA was superior to that of MFE. The study findings warrant verification. This trial was registered under identifier ChiCTR-IPR-15006370.

В настоящее время для лече-ния вертеброгенной боли используют фармакологические и немедикаментозные методы лечения. Вместе с тем в ряде случаев применение этих методов противопоказано или не-эффективно. В этой связи актуален поиск новых методов те-рапии болевого синдрома. Известно, что аргинин-вазопрес-син (АВП) наряду с гормональными эффектами проявляет свойства нейромодулятора. В эксперименте показано, что АВП проявлял анальгетические свойства. АВП уменьшал выраженность боли у больных с почечной коликой, с мигре-нозными приступами, болями вследствие онкологических заболеваний [24,36,37]. Данных о влиянии АВП на вер-теброгенный болевой синдром нет. В этой связи целью ра-боты была оценка эффективности агониста V2-рецепторов АВП, 1-дезамино-8-D-аргинин-вазопрессина (ДДАВП) в коррекции болевого синдрома у больных с ДДЗП и ко-решковым синдромом. 23 больным с вертеброгенными бо-лями ДДАВП вводили интраназально капельно в суточной дозе 1·10 -7 г. Контрольная группа состояла из 10 боль-ных со спондилогенной болью, которые получали тради-ционную терапию. Лечебный эффект ДДАВП наблюдался в 87 % случаев. У больных с умеренной и слабой болью ее выраженность снизилась через 2,5-3 часа от начала лече-ния. Наряду с купированием боли, у больных регрессиро-вали астено-депрессивные нарушения. Таким образом, по-казано, что ДДАВП был эффективен и безопасен в лечении вертеброгенных болей. AAdsoras.Pain is the leading symptom with degenerative-dystrophic diseases of the vertebral column. Now for the treatment of vertebral pain using pharmacological and non-pharmacological treatments. However, in some cases, the application of these techniques is contraindicated or ineffective. In this regard, relevant search for new methods of therapy of pain syndrome. It is known that argininevasopressin (AVP), along with the hormonal effects shows properties of neuromodulator. The experiment shows that AVP showed analgetic properties. AVP reduced the severity of pain in patients with renal colic, with migraine, pains due to cancer [24,36,37]. There is not data on the impact of AVP on vertebrogenic pain syndrome. In this context, the aim of the research was to assess the effectiveness of the agonist V2-receptors, 1-dezamino-8-D-arginine-vasopressin, DDAVP, in correction of pain syndrome in patients with degenerative-dystrophic diseases of the vertebral column. Treatment received 23 patients. DDAVP is injected intranasal drip in a daily dose of 1•10 -7 g. The control group consisted of 10 patients with vertebral pain who received traditional therapy.
