Aims: This study aimed to classify alcohol-dependent outpatients on the basis of clinical factors and to verify if the resulting types show different treatment retention. Methods: The sample comprised 332 alcoholics that were enrolled in three different pharmacological trials carried out at São Paulo University, Brazil. Based on four clinical factors – problem drinking onset age, familial alcoholism, alcohol dependence severity, and depression – K-means cluster analysis was performed by using the average silhouette width to determine the number of clusters. A direct logistic regression was performed to analyze the influence of clusters, medication groups, and Alcoholics Anonymous (AA) attendance in treatment retention. Results: Two clusters were delineated. The cluster characterized by earlier onset age, more familial alcoholism, higher alcoholism severity, and less depression symptoms showed a higher chance of discontinuing the treatment, independently of medications used and AA attendance. Participation in AA was significantly related to treatment retention. Discussion: Health services should broaden the scope of services offered to meet heterogeneous needs of clients, and identify treatment practices and therapists which improve retention. Information about patients’ characteristics linked to dropout should be used to make treatment programs more responsive and attractive, combining pharmacological agents with more intensive and diversified psychosocial interventions.

Extended-release naltrexone (XR-NTX; Vivitrol), developed to address poor adherence in addictive disorders, is approved for use in alcohol and opioid-dependence disorders. In alcohol-dependent adults with ≥ 4-day initial abstinence, XR-NTX increased initial and 6-month abstinence. An fMRI study found that XR-NTX attenuated the salience of alcohol visual and olfactory cues in the absence of alcohol, and post hoc analyses demonstrated efficacy even during high cue-exposure holiday periods. Safety and tolerability have generally been good, without adverse hepatic impact or intractable acute pain management. XR-NTX use appears feasible in primary care and public systems, and retrospective claims analyses have found cost savings and decreased intensive service utilization relative to oral agents. In opioid dependence, following detoxification, XR-NTX shows efficacy for maintaining abstinence, improving retention, decreasing craving, and preventing relapse. Trials are also exploring its use for the treatment of stimulant dependence. XR-NTX appears compatible with counseling and self-help attendance. While more research is needed, current findings suggest that a formulation of naltrexone that was sought beginning over three decades ago is fulfilling its promise as an extended-release pharmacotherapeutic.

RESUMOEstatísticas recentes mostram que milhões de brasileiros apresentam dependência de álcool. Uma das principais estratégias para o tratamento desses dependentes é a terapia farmacológica, que ainda apresenta baixas taxas de sucesso. Nesse contexto, é de suma importância que grupos de pesquisa nacionais conduzam estudos de avaliação de fármacos para o tratamento de dependentes de álcool. Este estudo é uma revisão bibliográfica narrativa crítica realizada na base de dados MEDLINE que teve por objetivo identificar os grupos de pesquisa brasileiros interessados no tratamento farmacológico do alcoolismo. Foi realizada busca de todos os artigos publicados a partir de 1996 com a seguinte combinação de termos: alcoholism, drug therapy, humans e Brazil. Nove artigos foram localizados, sendo sete ensaios clínicos, uma revisão e um relato de caso. Esses estudos foram conduzidos em quatro instituições diferentes, sendo que uma foi responsável por mais da metade dos artigos. Considerando as publicações no MEDLINE sobre quimioterapia do alcoolismo em humanos, esses artigos representariam 0,71% da produção científica mundial no mesmo período. Os estudos envolvem cinco fármacos, sendo três aprovados para tratamento do alcoolismo -dissulfiram, naltrexona e acamprosato; e dois em avaliação -topiramato e gabapentina. Conclui-se que os pesquisadores brasileiros estão contribuindo efetivamente para a análise do arsenal terapêutico do tratamento do alcoolismo, assim como sua expansão com novos fármacos. Entretanto, o número de grupos de pesquisa nacionais interessados por esta questão ainda é muito pequeno para a extensão do problema no País.Palavras-chave: alcoolismo/quimioterapia; ensaio clínico; Brasil. ABSTRACTRecent statistics show that millions of Brazilians are alcohol dependents. One of the main strategies for the treatment of such dependents is pharmacological therapy, which presents low success rate. In such context, it is essential that national research groups lead analyses of existing and new drugs for the treatment of alcohol dependents. This study is a narrative and critical bibliographic review of MEDLINE database aiming to identify Brazilian research groups interested in pharmacological treatment of alcoholism. A search of all articles published since 1996 was performed with the following combinations of terms: alcoholism, drug therapy, humans and Brazil. Nine articles were found, including seven clinical trials, one review and one case report. Such studies were conducted by four distinct institutions, one of them being responsible for more than half of the papers. Considering MEDLINE publications on drug therapy of alcoholism in humans, these papers represent 0.71% of the worldwide scientific production during the same period. The studies comprise five substances, three approved for alcoholism treatment -disulfiram, naltrexone, and acamprosate; -and two under evaluation -topiramate and gabapentin.
