The role of a multidisciplinary pre-assessment clinic in reducing mortality after complex orthopaedic surgery

Abstract: INTRODUCTION This paper describes an audit loop. The aim of this study was to audit the effect of a specialised preoperative anaesthetic assessment clinic after hip and knee arthroplasty and revision arthroplasty. PATIENTS AND METHODS We studied patients undergoing hip and knee surgery (arthroplasty and revision arthroplasty). We collected data concerning postoperative admissions to the high dependency unit (HDU), intensive care unit (ICU) and post-anaesthesia care unit (PACU) (planned and unplanned rates of a… Show more

“…Patients with CAD or CRF were 2‐fold and 3‐fold more likely to be readmitted, respectively. This is similar to previous surgical studies and in accord with others demonstrating that rehospitalization of surgical patients is often for complications related to preexisting medical conditions . To prevent such readmissions, many institutions have implemented preoperative optimization clinics, which have shown efficacy in otolaryngology patients .…”

“…Therefore, evaluating patients in a multidisciplinary clinic prior to performing TJA is crucial in order to identify comorbidities and manage them, if required. These assessments significantly reduce postoperative mortality and pre-admission costs in complex orthopaedic surgeries, including TJA (29).…”

“…The incidence of blood transfusion was a median of 6.2% (95% confidence interval 2.9-11.4%) with a 30 day readmission of 0.7% (0.0-3.8%) an inpatient mortality of and in-hospital mortality was 0.0 (0.0-7.0)%. Patient satisfaction was high with a 98% percentile Press Ganey satisfaction score, providing a solid foundation for the PSH practical use [50,51] . Before surgery, the anaesthetic team oversees clinical testing.…”
