BackgroundThe baseline data pertaining to the national epidemiological survey of infectious keratitis remain scarce in China, and currently there is no corneal blindness control strategy developed by the nation.MethodsGeographically defined cluster sampling was used to randomly select a cross-section of residents from representative urban and rural populations in Hubei Province. Participants were selected from village registers, followed by door-to-door household visits. The assessment items included a structured interview, visual acuity testing, external eye examination, and anterior segment examination using slit lamp. Causes and sequelae of corneal disease were identified according to uniform customized protocol.ResultsThe prevalence of presenting corneal diseases was 0.8% (211/26 305), while the prevalence of infectious keratitis was 0.148% (39/26 305). The prevalences of viral, bacterial, and fungal keratitis were 0.065, 0.068, and 0.015%, respectively. There were no significant differences found between the prevalences of viral (accounting for 43.6%) and bacterial (accounting for 46.2%) corneal ulcers. cases of Acanthamoeba keratitis were not found. Infectious keratitis was the leading cause of corneal blindness (85.7%), and the prevalence of blindness in at least one eye resulting from infected corneas was 0.091% (95% CI: 0.067-0.127%).ConclusionsViral and bacterial mechanisms constitute the most important risk factors for infectious corneal ulcers in Central China. To reduce the rate and severity of infectious keratitis, he public health care policy should be focused on designing cost-effective strategies and operational programs for the prevention and prompt treatment of infectious corneal ulcers.

Aims To determine the prevalence and identify associated risk factors for pterygium in an elderly Mongolian population at high altitude in Henan County, China. Methods A population-based survey was conducted from June 2006 to September 2006. A stratified, clustered, random sampling procedure was used to select 2486 Mongolian people aged 40 years and older. Pterygium was diagnosed and graded clinically as grade 1 (transparent), 2 (intermediate), and 3 (opaque). Risk factors associated with pterygium were evaluated with logistic regression models. Results From 2486 eligible subjects, 2112 (84.9%) were examined. There were 378 people with either unilateral (n ¼ 228) or bilateral (n ¼ 150) pterygia, equivalent to an overall prevalence of 17.9% (95% confidence interval (CI) 16.3, 19.5). The prevalence increased with older age (v 2 -test of trend Po0.001). Visual acuity decreased with higher group of pterygium (v 2 ¼ 97.759, Po0.0001). Pterygium was independently associated with Schirmer's test (p5 mm) (odds ratio (OR) 2.4; 95% CI, 1.9, 3.1), tear breakup time (p10 s) (OR 2.3; 95% CI, 1.8, 2.9), lower education level (o3 years) (OR 2.1; 95% CI, 1.4, 3.2), increasing age (OR 2.0; 95% CI, 1.4, 2.8) for persons 70-79, compared with 40-49, and other risk factors. Conclusions The prevalence of pterygium in an older Mongolian population at high altitude is high, primarily because of ocular sun exposure and the other effects of the unique plateau climate, and representing an important health problem. People should be strongly encouraged to wear a wide-brimmed hat and/or sunglasses whenever they are outside.

BACKGROUNDPterygium is a degenerative and hyperplastic condition of conjunctiva. In this, subconjunctival tissue proliferates as vascularised granulation tissue, thus invading the cornea and destroying the superficial stroma and Bowman's membrane. It varies from small asymptomatic lesion to large, rapidly growing lesion, thus leading to impairment of vision. Pterygium occurs more frequently between 30 degrees latitude north and south of the equator called as "pterygium belt." The most challenging situation after pterygium excision is the recurrence of the pterygium. Many techniques had been used, but none of them is universally accepted due to variable recurrence rate.The aim of the study is to compare the two modalities of treatment in pterygium excision with postoperative outcomes.
