Summary Two hundred and thirteen lung tumours of primary site and 42 metastases were heterotransplanted into nude mice with an overall success rate of 44%. There were differences in success between the histological types. Squamous cell and adenocarcinoma had the highest success rate (51% and 43%, respectively) whereas large cell and small cell carcinoma had a lower success rate (38% for both). The average volume doubling times in the first passage in nude mice ranged from 8.2 in large cell carcinomas to 18.9 days in adenocarcinomas. In subsequent passages an increase in growth rate was found, the overall average doubling time falling from 14.5 days in the first passage to 7.1 days in the second passage. In a study with 171 non-small cell lung carcinomas (NSCLC), the growth data in nude mice were correlated with the clinical data of the corresponding patients. A relationship between the growth parameters in nude mice and prognosis of patients could not be found.During the past 3 years we have transplanted in the course of cooperative clinical studies 255 human lung tumours into nude mice. In this report we describe the incidence and rate of growth of these tumours in the nude mouse, as primary transplants and passaged tumours, and further we examined whether the growth characteristics of these tumours might be of clinical prognostic value. Comparison of a part of the presented data with other measurements and the clinical course will be published elsewhere. Materials and methodsNude mice Athymic nude NMRI-mice (own breeding or Zentralinstitut fur Versuchstierzucht, Hannover), female, 6-9 weeks old, were kept under standardized conventional conditions (Makrolon cages, 270C room temperature, 50% relative humidity, autoclaved bedding) in a separated room especially controlled against infections. An autoclaved special diet (altromin 1410, Altromin Spezialfutterwerke, D-4937 Lage) and acidified water (pH 2-3) were given ad libitum. Patients and tumoursTwo hundred and fifty five histologically verified lung tumours and their lymph node metastases from 213 patients were xenografted into nude mice.

Summary The activity of a new nitrosourea (TCNU) based on the endogenous amino acid taurine was assessed in three human lung cancer xenografts growing in immunodeficient mice. Moderate activity (specific growth delays of 0.63 and 1.13 compared with controls) was seen in two non-small cell tumours after a single oral administration of 20 mg -1kg. This dose was curative in a small cell xenograft. By using high performance liquid chromatography it was possible to detect parent drug in the tumours as well as the plasma and tissues after oral administration of TCNU. Drug sensitivity was correlated inversely with the amount of the DNA repair enzyme 06-methylguanine-DNA methyltransferase assayed from extracts of the tumour cells but not with the levels of parent drug within the tumour. This compound appears to have unique pharmacokinetic properties compared with other chloroethylnitrosoureas.

Les limites du traitement guidé par les données génomiquesL'établissement du profil génétique des tumeurs est aujourd'hui à portée de main, grâce aux rapides progrès du séquençage de nouvelle génération (NGS, new generation sequencing) et à son entrée progressive dans la pratique clinique [1]. Dans de nombreux centres, on commence à analyser l'ADN de tumeurs primaires ou de métastases en séquençant un panel de 50 à 250 gènes, ou, plus rarement, en lisant l'ensemble de l'exome. Les procédés de purification et de lecture sont bien au point, les coûts (deux à trois milliers d'euros) et les délais (moins d'un mois) sont supportables. De plus, les techniques d'analyse informatique des résultats ont beaucoup progressé. L'objectif de ces tests, générale-ment pratiqués pour des cancers avancés métastatiques et ne répondant plus aux traitements, est d'identifier des altérations sur lesquelles on pourra agir (actionable alterations) grâce à des thérapies spécifiques. Selon les équipes et les technologies utilisées (et selon aussi la définition plus ou moins large de ce que sont ces « alté-rations ciblables »), on en trouve effectivement chez la majorité des malades, et quelques résultats cliniques spectaculaires ont été rapportés. Mais il faut reconnaître que, pour le moment, cette approche séduisante n'a pas encore fait la preuve de son utilité clinique, à savoir la démonstration d'une amélio-ration statistiquement significative de l'évolution des patients. À cet égard, la récente publication des résultats de l'essai français SAFIR01 [2] a fait l'effet d'une douche froide [1]. Dans cet essai clinique multicentrique, qui avait inclus plus de 400 patientes atteintes de cancer du sein métastatique, une douzaine de gènes ont pu être analysés chez 300 d'entre elles, une altération pathogène a été trouvée chez près de 200, mais, au final, une thérapie personnalisée n'a pu être proposée que pour 55 malades (et effectivement réalisée chez 43). Cette importante déperdition est due principalement au fait que, pour beaucoup des altérations repérées, les traitements n'existent pas ou ne sont pas accessibles (par exemple, ils sont en cours d'essai clinique mais pour une autre indication). Et finalement, seules quatre patientes (sur les 43 traitées) ont présenté une réponse au traitement utilisé… Cet essai a néanmoins eu le mérite de démontrer la faisabilité pratique d'une telle approche, et de fournir des chiffres qui en quantifient l'efficacité -même si celle-ci se révèle nettement plus faible qu'espéré. Bien sûr, les techniques employées (hybridation génomique comparative et séquençage Sanger) datent un peu 1 , et on peut penser qu'une analyse portant sur un grand nombre de gènes grâce au NGS aurait un rendement plus élevé. C'est d'ailleurs l'objectif de l'essai SAFIR02 qui démarre et inclura cette fois le séquençage NGS d'un large panel de gènes. Une autre étude, menée à l'Université de Californie, vient de faire l'objet d'une publication [3] dont les résultats sont un peu plus positifs -mais l'effectif est bien moindre et le recu...
