The repertoire of seizure onset patterns in human focal epilepsies: Determinants and prognostic values

Abstract: Objective: In this study, we seek to analyze the determinants of the intracranial electroencephalography seizure onset pattern (SOP) and the impact of the SOP in predicting postsurgical seizure outcome. Methods: To this end, we analyzed 820 seizures from 252 consecutive patients explored by stereo-electroencephalography (total of 2148 electrodes), including various forms of focal refractory epilepsies. We used a reproducible method combining visual and time-frequency analyses. Results: We described eight SOPs:… Show more

“…Visual Analysis of SEEG Seizure onset zone (SOZ) was visually defined as the first clear SEEG change occurring prior to, or concomitant with, clinical seizure onset. Seizure onset patterns (SOPs) were classified according to Lagarde et al 21 The first "tonic" or fast activity at onset and within 1 second was analyzed in relation to intrainsular anatomical location of involved contacts. Mean values for seizure duration, intrainsular and extrainsular propagation, and first subjective and objective signs were calculated.…”

“…SOPs are heterogenous and similar to other brain regions. Only 2 of 12 patients (17%) had onset with LVFA, compared with 43% 32 to 46% 21 in SEEG cohorts of mixed etiology and lobar onset. Reduction in low frequencies and increase in fast activities occurs at seizure onset, 33 seen on time frequency analysis as an "epileptic fingerprint," 25 but this fingerprint also occurs at distant but closely connected functional areas, for instance pre-SMA and contralateral insula in some anterior insula seizures; therefore, this is not a biomarker for only the SOZ or "what to resect area."…”

“…For patients with >10 captured seizures, we reviewed all seizures, then analyzed 10 representative seizures, so as to keep the same proportion of each distinct electroclinical seizure type. Seizure onset was defined as the first unequivocal SEEG signal change from the background in the setting of a sustained rhythmic discharge …”

“…In a range of malformative pathologies, it is well established that the EZ can extend beyond visible lesions . Additionally, over the past decade, two types of spatial organizations of the EZ have been observed in various epilepsies: (1) a focal EZ limited to a lesional/restrained cortical area and (2) a more widespread network involving lesion and/or multiple distant structures. We propose that distinct EZ spatial organizations may also be observed in TSC.…”
