Highlights Selected studies are heterogeneous in terms of patient characteristics as well as type, duration and frequency of rehabilitative approach. Neuromotor rehabilitation promotes neuroplasticity, as measured with MRI. Advanced MRI techniques provide reliable markers of structural and functional connectivity that may potentially aid in helping to implement the most appropriate rehabilitation intervention.

Segundo a Organização Mundial de Saúde (OMS) o Acidente Vascular Encefálico (AVE) é uma das causas que mais leva a morbidade e mortalidade no mundo. A Terapia por Contensão Induzida (TCI) é uma técnica de reabilitação derivada da neuropsicologia que tem como objetivo recuperar a função do membro superior acometido por uma lesão. Prática mental (PM), também conhecida como Imagética Motora (IM), consiste no método de treinamento em que a reprodução interna de um determinado evento é repetida extensivamente com a intenção de aprender uma nova habilidade ou melhorar o desempenho de uma tarefa já conhecida. O objetivo do presente estudo foi comparar as técnicas de TCIM e IM em relação a seus benefícios como ferramenta de reabilitação na melhora do desempenho da disfunção do membro superior decorrentes de AVE. Trata-se de uma revisão sistemática da literatura realizada no período de março a abril 2020. Para a busca utilizaram-se as bases de dados PEDro, Lilacs, Medline e Pubmed de artigos no idioma português, inglês e espanhol, publicados de 2015 a 2020, com as palavras-chaves Constraint-induced movement therapy, Stroke, Upper Limb/ Upper Extremity, Imagery Motor e Mental Practice e suas respectivas correspondências em português. Na busca eletrônica pelos bancos de dados Lilacs, Pedro, Medline e Pubmed, cruzando as palavras descritas no método, foram encontrados 526 artigos, destes foram selecionados 217 artigos para leitura na íntegra. Os resultados desta revisão sistemática indicam que ambas as técnicas foram igualmente capazes de proporcionar benefícios na melhora do desempenho da disfunção do membro superior decorrentes do AVE. Embora, os melhores resultados foram observados quando associadas a outras técnicas ou quando combinadas.

Background. Reorganization in motor areas have been suggested after motor imagery training (MIT). However, motor imagery involves a large-scale brain network, in which many regions, andnot only the motor areas, potentially constitute the neural substrate for MIT. Objective. This study aimed to identify the targets for MIT in stroke rehabilitation from a voxel-based whole brain analysis of resting-state functional magnetic resonance imaging (fMRI). Methods. Thirty-four chronic stroke patients were recruited and randomly assigned to either an MIT group or a control group. The MIT group received a 4-week treatment of MIT plus conventional rehabilitation therapy (CRT), whereas the control group only received CRT. Before and after intervention, the Fugl-Meyer Assessment Upper Limb subscale (FM-UL) and resting-state fMRI were collected. The fractional amplitude of low-frequency fluctuations (fALFF) in the slow-5 band (0.01-0.027 Hz) was calculated across the whole brain to identify brain areas with distinct changes between 2 groups. These brain areas were then targeted as seeds to perform seed-based functional connectivity (FC) analysis. Results. In comparison with the control group, the MIT group exhibited more improvements in FM-UL and increased slow-5 fALFF in the ipsilesional inferior parietal lobule (IPL). The change of the slow-5 oscillations in the ipsilesional IPL was positively correlated with the improvement of FM-UL. The MIT group also showed distinct alternations in FCs of the ipsilesional IPL, which were correlated with the improvement of FM-UL. Conclusions. The rehabilitation efficiency of MIT was associated with increased slow-5 oscillations and altered FC in the ipsilesional IPL. Clinical Trial Registration. http://www.chictr.org.cn .
