The reliability of the mankin score for osteoarthritis

Abstract: For the histopathological classification of the severity of osteoarthritic lesions of cartilage, the Mankin score is frequently used. A necessary constraint on the validity of this scoring system is the consistency with which cartilage lesions are classified. The intra- and interobserver agreement of the Mankin score was determined. The intra- and interobserver agreement of the 14-point Mankin score was adequate. Between observers 95% of differences were less than approximately 7 points. By a more strict defin… Show more

“…After 2 weeks in fixative (with multiple changes), the samples were placed in formic acid-formaldehyde decalcification solution (Cal-EX II; Fisher Scientific) for a further 2 weeks. Decalcified samples were paraffin embedded and sectioned at 6 m, and the mounted slides were stained with either toluidine blue or Safranin O to assess cartilage health using a modified Mankin scoring system (21). Slides were assessed in a randomized manner by 2 independent observers (MDJ and GL) who were blinded with regard to study group, and the average of the 2 scores was used for analysis.…”

“…21 The severity of cartilage damage of each joint was assessed using the modified Mankin grading system. 22 For each limb, mid-sagittal sections were evaluated from the medial and lateral condyles of the distal femur and proximal tibia. Three independent observers scored each section, and the scores for the medial and lateral tibial and femoral condyles were averaged within each joint.…”

“…Frontal sections (6 pm) were cut using a rotary microtome, and stained using safranin-O/fast green or haematoxylin and eosin. A modified Mankin grading system [26] was used to characterize the superficial surface structure, cellularity, safranin-0 staining characteristics, and tidemark integrity [21]. When examining the histology, the single observer was blinded as to the treatment.…”
