The relative efficacy of topical non-steroidal anti-inflammatory drugs and capsaicin in osteoarthritis: a network meta-analysis of randomised controlled trials

Abstract: SummaryObjectiveTo compare the efficacy of topical non-steroidal anti-inflammatory drugs (NSAIDs) with topical capsaicin for pain relief in osteoarthritis (OA).DesignA systematic literature search was conducted for randomised controlled trials (RCTs) examining any topical NSAID or capsaicin in OA. Pain relief at or nearest to 4 weeks was pooled using a random-effects network meta-analysis (NMA) in a Frequentist and Bayesian setting. Analysis was conducted for all trials and for trials using drugs listed as lic… Show more

“…However, little is known about the pathogenesis of OA pain, and thus, there is a lack of targeted treatment and measures to control OA pain. Nonsteroidal anti-inflammatory drugs [3,4] and opioids [5,6] have a certain effect on relieving OA pain, but longterm use of these drugs has great side effects, and they cannot fundamentally reverse or delay the progression of the disease [7][8][9][10]. Although surgery is effective in relieving pain in patients with advanced OA, surgery itself can cause pain and increase the financial burden on the patient.…”

“…Topical NSAIDs and capsaicin may be beneficial. 18,19 Regarding complementary medicines, glucosamine and chondroitin sulphate are no more effective than placebo. 20,21 Fish oils and turmeric have mild anti-inflammatory effects, but there is insufficient evidence to support their use.…”

“…Um systemische Nebenwirkung generell geringer zu halten, ist bei Patienten mit peripherer Arthritis und Osteoarthrose auch die lokale Behandlung mit Ketoprofen-oder sonstigen NSAR-haltigen Präparaten eine wichtige Alternative [48,49] und wird z.T. sogar als Erstlinientherapie empfohlen [50][51][52][53].…”
