T hyroid cancer has an increasing incidence due to the increase in diagnostic imaging modalities. Papillary thyroid carcinoma (PTC) is the most common differentiated thyroid cancer (DTC). [1] However, the etiology has not been clarified much, except for reasons such as age, a history of radiation to the neck region, and a previous history of PTC, which predispose to DTC. [2] Thyroid-stimulating hormone (TSH) is the main regulator and growth factor of the thyroid. Since it is the major hormone stimulating thyrocytes, increased serum concentrations of TSH have been found to be associated with cancer risk in thyroid nodules. [3,4] The basis of this view is that TSH has an effect on the proliferation of malignantly transformed DTC cells, as well as on thyrocytes. TSH has been Objectives: Thyroid-stimulating hormones (TSHs) are associated with the risk of differentiated thyroid cancer. The relationship between pre-operative TSH levels and aggressive features is unclear. We aimed to evaluate the relationship between pathological features of papillary thyroid carcinoma (PTC) and high TSH levels. Methods: Patients who were operated between 2012 and 2017 and who were found to have PTC in their pathology were included in the study. The relationship between TSH and the features of tumor aggressiveness was evaluated in the patients. Results: Of the 132 patients, TSH level was significantly higher in those with lymphovascular invasion than those without (p=0.048), in those with central metastases than in those without (p=0.014), and in those with extrathyroidal spread than in those without (p=0.003). When patients were categorized into four 25% quartiles according to TSH (mUI/mL) level; the rate of extrathyroidal invasion increased as the TSH level increased, and the level was significantly higher in quartile 1 than the others, with significant difference (p=0.030). Conclusion: Pre-operative increase in TSH level is associated with an increased risk of extrathyroidal spread and central lymph node metastasis. TSH level may be a pre-operative valuable predictive factor for patients' risk of central metastasis.

Background: Transoral endoscopic thyroidectomy has been gaining popularity as it allows patients to avoid a visible scar in the neck. However, there are still exiting disadvantages of traditional transoral endoscopic thyroidectomy，including numbness in the lower jaw or excessive edema. Therefore, we designed and applied a novel Mini transoral endoscopic thyroidectomy on patients requiring for thyroidectomy.Patients and Methods: A total of 55 patients requiring for unilateral thyroidectomy who underwent novel Mini transoral endoscopic thyroidectomy (Mini group) were identified from August 2019 to December 2019. As a comparison, a matched cohort of 55 patients requiring for conventional transoral endoscopic thyroidectomy was also identified. The two groups were analyzed in terms of clinical characteristics, perioperative/operative clinical date and post-operative complications. Results: The hospitalization time of patients in the Mini group was obviously less than that in traditional group. Furthermore, the numbness of the lower jaw, nylon wire examination score, pressure needle puncture examination score, or numbness VAS score indicated that the degree of numbness of the jaw in Mini group is significantly better than the traditional group. Moreover, the pain of the axillary incision using the pain VAS score method also showed that Mini group is significantly better than the traditional group. The results of pain of the axillary incision using the VAS score method also showed that novel Mini group was significantly better than that in the traditional group. Furthermore, dysfunction of mandibular muscles was observed in six of fifty-five patients in the traditional group, whereas no dysfunction of mandibular muscles observed in Mini group. At last, the degree of postoperative mandibular edema in Mini group was markedly slighter than that in traditional group. The post-operation infection rate was markedly lower in Mini group.Conclusion: The new novel Mini transoral endoscopic thyroidectomy is safe and feasible, and reduces the damage to the functional muscles such as the mandibular diaphragm and lower lip muscles, which significantly reduces the numbness and edema of the mandible after surgery. Our novel Mini transoral endoscopic thyroidectomy is a feasible and safe advancement, which can be a viable alternative protecting patients from insufferable numbness and pain.

In an assessment of risk for differentiated thyroid cancer (DTC) in individuals with human papillary thyroid cancer (PTC) and thyroid nodules a cohort prospective study was undertaken to establish the significance of preoperative thyroid-stimulating hormone (TSH) levels. Confirmed histologically PTC cases in one tertiary care center, and matched healthy individuals were tested for TSH, T3, T4 and T4 free total. The ORs and 95% confidence intervals have been calculated using conditional logistic regression models (CI). The blood TSH levels were related to the higher risk of PTC for men (OR,0,09; 95% Ci, 04–0,21, 95% CI and women) compared with the middle tertile of the TSH levels in the normal range (OR,0,07; 95 percent CI, 0,04–0,1). Over the normal range of TSH levels, an elevated PTC risks were connected amongst women (OR 0,09; 95% CI, 0,04–0,21) but not amongst men (OR,0,07; 95% CI, 0,04–0,1). With an increase in TSH level in the normal range between men and women, the risk for PTC reduced (Ptrend=0.041 and 0.0001). The risk of PTC related to TSH levels has been dramatically elevated above the normal range for men and TSH values below the normal range for women.
