The Relationship Between the Angiographic Findings and the Clinical Features of Carotid Artery Plaque

Abstract: The purpose of this study was to investigate the histological characteristics of atheromatous carotid plaque, and to analyze the relationship between the angiographic findings and the clinical features. We retrospectively reviewed 55 cases of carotid endarterectomy for extracranial internal carotid artery stenosis, who were treated at our institute from January 1995 to December 1997. The histological examination included hematoxylin-eosin staining, Masson-trichrome staining, and immunostaining for antismooth m… Show more

“…Thirty-nine studies used carotid ultrasound* as the imaging modality, 4 used intra-arterial angiography, 31,56,60,77 7 used computer tomography, 37,41,45,64,75,78,79 and 30 used magnetic resonance imaging. † Three studies used other forms of imaging 24,26,89 and some studied more than one imaging modality.…”

“…In the NASCET study, in the group of patients who received medical therapy, 30% of patients who had a severe carotid stenosis associated with an ulcerated plaque suffered an ischemic cerebral event within 2 years, whereas only 17% of patients with severe stenosis but no ulcerated plaques had an ischemic cerebral event within 2 years. 4 The presence of ulceration alone represents an important risk for neurologic symptoms, 7 and a high-grade stenosis, combined with plaque ulceration, produces an increased risk of stroke. 4,8 An accurate delineation of plaque morphology is therefore very important.…”

“…4,8 An accurate delineation of plaque morphology is therefore very important. 7,8 Incidence of ulceration in the carotid artery is variable, ranging from 14% to 68%, [9][10][11][12] and plaque ulceration is significantly more frequent in symptomatic patients. 11,13 Plaque ulceration has been defined as "an intimal defect larger than 1000-m in width, exposing the necrotic core of the atheromatous plaque."…”

“…Of 4 published studies that have reported some comparison with histological assessment, 21-24 2 provide only qualitative results, 23,24 and 2 reported only comparisons between angiographic ulceration and histological plaque rupture. 21,22 We have therefore performed the largest study of this kind (128 carotid plaques) to determine which of 10 histological features are associated with assessments of plaque ulceration on angiography. We have also investigated the histological significance of plaque surface irregularity and ulcers of different types.…”
