The relationship between renal volume and renal function in autosomal dominant polycystic kidney disease

Abstract: This study demonstrated a significant relationship between the change in renal function and the change in renal volume in Japanese ADPKD patients without renal insufficiency. It is possible that the volume measurements can be used as useful markers for disease progression in Japanese ADPKD patients.

“…showed that the combination of IMT and ABI could have an additive effect for predicting CVD, also if in our study only ABI and not IMT results correlated with TFV. However, Tokiwa et al . showed that in ADPKD patients with preserved renal function, ABI found no significant relationship with disease progression, while in our study ABI was significantly correlated with TFV.…”

“…Patients with ADPKD and higher kidney volumes are at higher risk for more rapid progression to ESRD relative to patients with more modest kidney volumes. (3, 26) Assuming that tolvaptan reduces the rate of eGFR decline by attenuating cyst growth, the TEMPO trial may have overestimated the therapeutic benefit of tolvaptan to the ADPKD population at large. Indeed, in a cohort of patients with ADPKD of similar age and eGFR but with no kidney volume criterion for inclusion, the mean kidney volume was 562 ml and the average rate of eGFR decline was –2.4 ml/min/1.73m 2 , in contrast to 1,693 ml and –3.7 ml/min/1.73m 2 , respectively, in TEMPO.…”

“…Indeed, endothelial dysfunction is an early and potentially reversible manifestation of atherosclerosis, with multifactorial etiology [52-54], and it develops in ADPKD patients, preceding the renal failure [55-56]. Atherosclerosis has been considered to play a key role in the early phases of cardiovascular disease found in ADPKD [57-59]. Cardiopulmonary exercise testing is considered the gold-standard for assessment of cardiorespiratory fitness.…”
