Pain from injections is common in children of all ages, and more than 90% of hospitalized children experience invasive and painful procedures such as venipuncture. In light of the complications associated with pain relief medications, nonpharmacological and complementary medicine approaches have gained attention. This study aims to compare the effects of acupressure and music on venipuncture pain intensity in children. This randomized controlled clinical trial involved 180 children aged 3–6 years who sought treatment at the Children’s Medical Center Hospital Emergency Department at Tehran University of Medical Sciences, Iran. The children were randomly assigned to one of three groups: acupressure, music, or control. The interventions were given within 5 minutes, starting 3 minutes before the venipuncture and continuing until completion. The interventions included playing music through headphones and applying acupressure to the Hugo point. Venipuncture was carried out under identical conditions using an Angiocath 24G needle. Pain intensity was assessed using the Oucher scale. Data were analyzed using SPSS 24, employing the Kruskal–Wallis, chi-square, and Bonferroni pairwise comparison tests, with a significance level of 0.05. The mean pain intensity was 3.32 ± 1.44 in the music group, 4.82 ± 1.51 in the acupressure group, and 8.32 ± 1.10 in the control group. Pain intensity significantly differed among the three groups p<0.001. Specifically, pain intensity was lower in the music group compared to both the acupressure p<0.001 and control p<0.001 groups. Furthermore, pain intensity was lower in the acupressure group than in the control group p<0.001. Based on the results, music and acupressure methods effectively reduce pain intensity during venipuncture in children. Considering that music demonstrated a more pronounced effect in alleviating venipuncture pain than acupressure, the recommendation is to utilize music as a method of pain management during venipuncture in children. Background: As direct and continuous providers of patient services, nurses are more exposed tovarious types of psychological stress in the work environment compared to other members of the health system. Nurses working in pediatric wards are in contact with children, and their families are more exposed to moral distress.This study aimed to investigate Iranian nurses’ understanding of moral distress in pediatric wards. Methods: This qualitative study was conducted through inductive content analysis in 2023-2024. Data were collected through semi-structured and individual interviews and analyzed simultaneously and continuously using Graneheim and Lundman’s approach. Twelve participants among nurses with moral distress experiences were selected through purposive sampling from Babol and Tehran Universities of Medical Sciences. Results: The study findings include four main classes and eight subclasses: 1) moral distress related to colleagues (doctor and nurse colleagues), 2) moral distress related to parents (conflict with children’s rights and distrust in nurses), 3) moral distress related to organizational factors (lack of manpower and proper equipment), and 4) psychological tensions following moral distress (mental conflict and exhaustion). Conclusion: The findings demonstrate the need for mechanisms by managers and officials to hold educational programs and workshops to help reduce moral distress levels in nurses. To this end, it is necessary to identify the stressful conditions caused by nursing care in this ward and reduce nurses’ exposure to stress in moral dilemmas.

The Effect of Endotracheal Suctioning Using the Four-handed Care on Physiological Criteria and Behavioral Responses of the Preterm Infants: Randomized Crossover Clinical Trial

Introduction: Endotracheal suctioning is one of the procedures that can cause pain and stress for infants admitted to neonatal intensive care units (NICUs). Pain and stress can be manifested with physiological and behavioral responses. This study is a crossover randomized clinical trial design that aimed to investigate the effect of endotracheal suctioning, using four-handed care on the physiological criteria and behavioral responses of preterm infants. Methods: In this study, 40 infants were randomly divided into two groups of 20, one group was first suctioned by the routine method (two hands) and then with the four-handed method. The other group was first suctioned by the four-handed method and then with the routine one. The ALPS-Neo (Astrid Lindgren Children’s Hospital Pain Scale Neonates) was used to evaluate pain and stress in infants. One camera recorded facial expressions and body movements, and physiological data were recorded from the monitor simultaneously. Results: Repeated measures analysis of variance (ANOVA) showed no statistically significant difference in changes in arterial saturation, heart rate and behavioral response scores between the two methods, but there was a significant difference in the mean heart rate of infants during and two minutes after suctioning in four-handed method using independent t test. Data were analyzed using SPSS version 13. Conclusion: Four-handed suctioning method can prevent an increase in the heart rate during and two minutes after suctioning, but it does not seem to affect behavioral responses and oxygen saturation of the infants. Since one of the symptoms of pain and stress in infants is the change of vital signs, especially the heart rate, we recommend four-handed method for suctioning of endotracheal tube.
