The Relationship between Hippocampal Volume and Cognition in Patients with Chronic Primary Insomnia

Abstract: Background and PurposeDifferences in hippocampal volume (HV) were compared between chronic primary insomniacs (PIs) and good sleepers (GSs), and the relationship between HV and memory function in PIs was investigated to clarify the effect of chronic sleep deprivation on brain structure and cognition.MethodsTwenty PIs (mean age, 50 years; 18 females) and 20 age-, gender-, and education-matched GSs were enrolled. Brain magnetic resonance imaging (MRI) was performed on a 1.5-T MRI scanner. Left and right HV and i… Show more

“…47 Noh et al found that the significant lower scores on the tests of attention and cognition function, especially for the frontal lobe function, were significantly worse in the PCPIs than in the GSs, which suggest that the memory deficit in the PCPIs might be directly influenced by impaired attention and frontal lobe function. 48 Joo et al found that the PCPIs showed significant reduction of gray matter concentrations in the dorsolateral prefrontal cortices compared with the GSs. 7 Our previous study also found the PCPI-Ms showed lower regional homogeneity in the dorsolateral prefrontal cortex compared with the GS-Ms. 23 In support of these findings, in the present study we found that the PCPI-Fs showed lower ALFF in the left dorsolateral prefrontal cortex compared with the GS-Fs.…”

“…One experienced neuropsychologist (Y.-J.K.) carried out all the NP tests including memory, attention/working memory, language, frontal lobe functions and mood. The detailed methodology for the standardized NP test has been described previously [16]. …”

“…Winkelman et al [82] followed this with a study of 20 patients with PI and 15 good-sleeping control patients in which they used manually defined hippocampal traces and found no difference in hippocampal volumes between the 2 groups. Noh et al [58] found that, in 20 individuals with PI, hippocampal volume was negatively correlated with duration of insomnia symptoms. In this study, manually measured hippocampal volumes did not differ statistically from matched controls, although subjects classified as having PI did not differ significantly in total sleep time or sleep efficiency.…”
