Background: Stroke has become a major problem around the world, which is one of the main causes of long-term disability. Therefore, it is important to seek a biomarker to predict the prognosis of patients with stroke. This meta-analysis aims to clarify the relationship between the neutrophil-to-lymphocyte ratio (NLR) and the prognosis of stroke patients.Methods: This study was pre-registered in PROSPERO (CRD42020186544). We performed systematic research in PubMed, Web of Science, and EMBASE databases for studies investigating the prognostic value of NLR. Based on the enrolled studies, patients were divided into the low-NLR cohort and the high-NLR cohort. Odds ratios (ORs) with 95% confidence intervals (CIs) were extracted and analyzed by the Review Manager 5.3 and Stata 12.0 software. Heterogeneity was estimated by using Cochran's Q test and I2 value. Sensitivity analyses and subgroup analyses were also performed to explore the potential sources of heterogeneity. Publication bias was assessed with funnel plots and assessed by Egger's tests.Results: Forty-one studies with 27,124 patients were included. In the overall analysis, elevated NLR was associated with an increased mortality in acute ischemic stroke (AIS) patients (OR = 1.12, 95% CI = 1.07–1.16) and in acute hemorrhagic stroke (AHS) patients (OR = 1.23, 95% CI = 1.09–1.39), poorer outcomes in AIS patients (OR = 1.29, 95% CI = 1.16–1.44), and in AHS patients (OR = 1.11, 95% CI = 1.03–1.20). While in terms of hemorrhagic transformation (HT), elevated NLR was associated with an increased incidence of HT in AIS patients (OR = 1.15, 95% CI = 1.08–1.23).Conclusions: This study demonstrated that elevated NLR was significantly associated with poor prognosis of stroke patients. High NLR is associated with a 1.1- to 1.3-fold increased risk of poor outcomes of AIS/AHS patients. Wake-Up Stroke (WUS) is defined as a stroke with the unclear onset of symptoms and subsequent neurological deficits which perceived upon awakening. WUS patients are often excluded from acute fibrinolytic and reperfusion therapy due to the unknown exact time of symptoms onset. This study aimed to evaluate patients with and without WUS characteristics and associated risk factors at two tertiary hospitals. First, we prospectively evaluated consecutive patients with stroke symptoms and determined stroke sub groups by using Computed Tomography (CT) scan. Next, demographic and clinical characteristics including past medical and drug consumption history as well as cardiac function index (ejection fraction), LDL (mg/dl) level and hematologic parameters: hemoglobin (Hb); hematocrit (Hct); platelet (Plt) were assessed. Results: 510 patients (56.1% men and 43.9% women) with averaged age of 70 and 72 years were studied, respectively. Of 510 patients, 405 (79.4%) had non-WUS stroke (known-onset stroke) and 105 (20.6%) had WUS strokes (unknown-onset stroke). The WUS occurrence most likely was observed in ischemic stroke compared to hemorrhagic one. No significant differences were found between patients from both groups regarding stroke risk factors. However, hypertension and family history were more common in patients with WUS (p>0.05). Moreover, individuals with a previous cerebrovascular accident in WUS group were almost similar to non-WUS counterparts. No differences also detected in case of hematologic characteristics, heart function index and LDL levels between study groups (p>0.05). Together, wake-up stroke occurs in approximately 20% of stroke subjects. In this study, patients with WUS had more hypertension and family history.

This study aims to interpret the hematological parameters of children with the intellectual disability and to show the relationship between hematological parameters and possible inflammatory mechanisms of intellectual disability. Materials and Methods: In this study, 50 children with the intellectual disability (32 males, 18 females) and 40 healthy individuals (25 males, 15 females) were retrospectively screened. Lymphocyte and platelet count, white blood cell count, neutrophil, platelet to lymphocyte ratio (PLR), neutrophil to lymphocyte ratio (NLR), red cell distribution width (RDW) and mean platelet volume (MPV) were recorded for both groups and compared. Results: Neutrophil to lymphocyte ratio and neutrophil counts in the intellectual disability group were elevated than control. Mean platelet volume in the intellectual disability group was higher than the control and red cell distribution width in children with the intellectual disability was elevated than control. We did not detect any statistical difference in neutrophil to lymphocyte ratio between the groups with or without inborn error of metabolism. Conclusion: The findings obtained in this study suggest that we can use mean platelet volume, neutrophil to lymphocyte ratio and red cell distribution width as inflammatory markers for the intellectual disability of children.
