Learning disabilities constitute a diverse group of disorders in which children who generally possess at least average intelligence have problems processing information or generating output. Their etiologies are multifactorial and reflect genetic influences and dysfunction of brain systems. Reading disability, or dyslexia, is the most common learning disability. It is a receptive language-based learning disability that is characterized by difficulties with decoding, fluent word recognition, rapid automatic naming, and/or reading-comprehension skills. These difficulties typically result from a deficit in the phonologic component of language that makes it difficult to use the alphabetic code to decode the written word. Early recognition and referral to qualified professionals for evidence-based evaluations and treatments are necessary to achieve the best possible outcome. Because dyslexia is a language-based disorder, treatment should be directed at this etiology. Remedial programs should include specific instruction in decoding, fluency training, vocabulary, and comprehension. Most programs include daily intensive individualized instruction that explicitly teaches phonemic awareness and the application of phonics. Vision problems can interfere with the process of reading, but children with dyslexia or related learning disabilities have the same visual function and ocular health as children without such conditions. Currently, there is inadequate scientific evidence to support the view that subtle eye or visual problems cause or increase the severity of learning disabilities. Because they are difficult for the public to understand and for educators to treat, learning disabilities have spawned a wide variety of scientifically unsupported vision-based diagnostic and treatment procedures. Scientific evidence does not support the claims that visual training, muscle exercises, ocular pursuit-and-tracking exercises, behavioral/perceptual vision therapy, “training” glasses, prisms, and colored lenses and filters are effective direct or indirect treatments for learning disabilities. There is no valid evidence that children who participate in vision therapy are more responsive to educational instruction than children who do not participate.

ПОГЛЯД НА ПРОБЕМУЦель работы -ознакомление практикующих врачей с проблемой недостаточности конверген-ции у младших школьников.Установлено, что у младших школьников суще-ствуют проблемы бинокулярного зрения, в частно-сти недостаточность конвергенции, выявление и диагностика которой требует расширение алгорит-ма обычного осмотра.Из-за преимущественного влияния на работо-способность вблизи и чтение диагностика данного состояния имеет большое значение для школьни-ков, особенно для имеющих проблемы с чтением и усидчивостью при выполнении домашних заданий. Учителя, психологи и родители должны быть ин-формированы о том, что таким детям необходима консультация офтальмолога. Учитывая высокую эффективность лечения недостаточности конвер-генции при постановке правильного диагноза, име-ется возможность существенно облегчить ребенку выполнение домашних заданий или дать соответ-ствующие рекомендации для снижения усталости зрительной системы. Кроме того, прицельное изу-чение работы аккомодационной и вергентной сис-тем позволяют определить подход к назначению коррекции рефракционных нарушений.Необходимо разработать алгоритм обследова-ния недостаточности конвергенции для практикую-щих врачей офтальмологических кабинетов, а так-же информировать педагогов, которые первыми могут обратить внимание на трудности с чтением, запоминанием прочитанного, неусидчивостью при выполнении заданий у школьников, о симптомах данного состояния.Ключевые слова: зрение, возрастные особен-ности, дети, педагогика, успеваемость.Введение. Недостаточность конвергенции (НК) -состояние, нередко встречающееся в пери-од возрастного становлення аккомодационно-вергентных отношений. НК проявляется обычно впервые в школьном возрасте -тогда, когда ребе-нок сталкивается с длительной работой на близ-ком расстоянии [1,2]. В дальнейшем проблема либо компенсируется, либо наоборот усугубляет-ся, в зависимости от зрительной нагрузки и ком-пенсаторных возможностей организма ребенка [3,4,5]. Усугубляют проблему недостаток сна, общие заболевания, стрессы [6].Обычное офтальмологическое исследование или скрининг с проверкой остроты зрения не по-зволяют адекватно выявить НК. Это состояние часто остается незамеченным еще и потому, что всестороннее исследование бинокулярного зрения не входит в алгоритм рутинного осмотра [7][8][9].Дети с НК могут иметь высокую остроту зрения и не иметь отклонений в рефракции, однако рабо-та зрительной системы у них нарушена и требует вмешательства. Objective-To compare home-based pencil push-ups (HBPP), home-based computer vergence/ accommodative therapy and pencil push-ups (HBCVAT+), office-based vergence/accommodative therapy with home reinforcement (OBVAT), and office-based placebo therapy with home reinforcement (OBPT) as treatments for symptomatic convergence insufficiency (CI).Methods-In a randomized clinical trial, 221 children 9 to 17 years with symptomatic CI were assigned to one of four treatments.Main Outcome Measures-Convergence Insufficiency Symptom Survey (CISS) score after 12 weeks of treatment. Secondary outcomes were near point of convergence (NPC) and positive fusional vergence at near (PFV).Results-After 12 weeks of treatment the OBVAT group's CISS score (15.1) was statistically significantly lower than the HBCVAT+, HBPP, and OBPT groups' scores of 21.3, 24.7, and 21.9, respectively (P < 0.001). The OBVAT group also demonstrated a significantly improved NPC and PFV compared with the other groups (P <= 0.005). A successful or improved outcome for the OBVAT, HBPP, HBCVAT+, and OBPT groups was found in 73%, 43%, 33%, and 35%, respectively.Conclusion-Twelve weeks of OBVAT results in a significantly greater improvement in symptoms and clinical measures of NPC and PFV and a greater percentage of patients reaching predetermined criteria of success when compared with HBPP, HBCVAT+, or OBPT.
