The relation between the arthroscopic findings and functional outcomes in patients with septic arthritis of the knee joint, treated with arthroscopic debridement and irrigation

Abstract: Advanced arthroscopic findings are associated with poor functional results in patients with septic arthritis of the knee joint. In addition, the time between the initial symptoms and the surgery directly affects the functional results.

“…The current series found a similarly high success rate (75%) after only 1 surgical session. Key differences between the studies are the inclusion of a greater number of patients in the current study treated with both open and arthroscopic methods and the use of a postoperative continuous irrigation-drainage system in the study by Yanmis et al 28 Prompt articular lavage of the septic knee is of utmost importance. In a study of 30 patients, most of whom received early treatment during a 72-hour interval, Smith 29 reported no recurrences or failures.…”

“…Critical aspects for successful treatment in septic knee arthritis include early treatment, joint decompression, appropriate systemic and oral antibiotics, and early mobilization. 12,13 Arthroscopic debridement leads to better functional outcomes and lower morbidity and is considered the appropriate treatment for a septic knee. Culture provides a definitive diagnosis at which point arthroscopic debridement and irrigation of the joint allows for debridement of necrotic synovium and irrigation of purulent enzymatically active material.…”

“…Joint aspiration of the knee is often for excess fluid, crystal analysis for gout/ pseudogout, blood, and septic joint. 12, 13 Injections of fluids include the following: pain medication/anesthetic injection after total knee replacement, 14 injection of viscosupplementation/ platelet-rich plasma/cryotherapy for osteoarthritis, 15 botulinum toxin injection for anterior knee pain, 16 knee irrigation/arthroscopic lavage procedures, 13 ultrasound-guided steroid injection for inflammatory arthritis, 17 calcium sulfate injection for tibial plateau fractures, 18 sclerotherapy of Baker cyst, 19 methotrexate for knee synovitis, 20 injection for meniscal cysts, 21 and intraarticular radioactive gold for knee effusions. 22,23 Aspiration of the Knee The knee may be aspirated to remove excessive joint fluid or blood (Figs.…”
