The rehabilitation of the severely resorbed maxilla by simultaneous placement of autogenous bone grafts and implants: a 10-year evaluation

Abstract: The long-term success of Brånemark implants has been previously reported. The success rate of the same implants, when associated with autologous grafts, seemed much less predictable. In the present study it is demonstrated that when these implants are installed in conjunction with an autologous corticocancellous hip graft, either onlay or inlay, the cumulative success rate remains at 95% for individual implants in non-smokers. The short extracorporal time might be an explanation. On the other hand, the same ap… Show more

“…This is in accordance with a 15-year follow-up by JeMT and JOHANSSON 14 who found similar long-term success rates after implant treatment in the edentulous maxillae in nongrafted patients. A survey of the literature on similar grafting technique as in the present study, reports survival rates varying from 49 to 95% 15,22,26,27,29,32 . The time in long-term follow-up studies varies from 12 to 124 months 26 ; only one study covered a minimum of 10 years 22 .…”

“…25,26 Soft tissue closure poses a limitation for reconstruction of the severely atrophic maxilla. In a review article, 22 partial loss of the graft because of wound dehiscence/infection occurred in 3.3% of the cases, whereas total loss of the graft occurred in 1.4% of the cases, 27,28 the majority being related to extensive reconstructions of the atrophic maxillae with iliac grafts. The overall survival rate of implants placed in conjunction with onlay bone graft was 79.5% (range, 60%-100%).…”

“…While the success rates of the sinus floor elevation vary between 76 and 100% and 48 and 100% of all alveolar crest onlay augmentations seem to be successful concerning implant survival. In these studies, however, it is difficult to compare the outcome of the applied techniques: Primary and secondary implant insertion were carried out, autogenous bone and bone substitutes were used, and different periods of observation have been compared with different methods 3,7,9,19,20,22,23,25,26 .…”
