The recommended treatment algorithms of the BCLC and HKLC staging systems: does following these always improve survival rates for HCC patients?

Abstract: Both the BCLC and HKLC staging systems effectively stratified patient prognosis, but neither could direct therapy for a large proportion of patients; for some stages, recommended therapy was associated with worse prognosis.

“…However, some systems are not consistent across all stages; for example, CLIP was more discriminating in early (score 0-3) than later (score 4-6) disease stages [51]. Survival was also found to be better when patients received recommended treatment in stages HKLC I, IIa/b, IIIa, or Va than in stages IIIb, IVa/b, or Vb, and when patients received recommended treatment in BCLC stages 0 or A, but not in stages B, C, or D. In addition, neither system (HKLC or BCLC) could direct therapy for a large group of patients [53].…”

“…Studies suggest that the BCLC system can predict prognosis more accurately for Caucasian HCC patients than Asian ones, while the converse may be true of the HKLC system [811]. Though both staging systems are comprehensive, many patients do not fall neatly into their pre-specified treatment pathways [11], and even patients within the same BCLC or HKLC stage can differ substantially.…”

“…Though both staging systems are comprehensive, many patients do not fall neatly into their pre-specified treatment pathways [11], and even patients within the same BCLC or HKLC stage can differ substantially. These treatment gaps highlight the need to assess actual tumor stages and primary treatments of HCC in large patient populations.…”

“…However, in clinical practice, resection, transplantation or local ablation is infeasible in many cases [10]. For hepatic resection, hepatic reservoir function and future liver volume or remnant liver volume after resection should be considered in order to prevent postoperative hepatic insufficiency [1].…”

“…We screened Samsung Medical Center HCC registry for the period from January 2007 to December 2012. Detailed description of Samsung Medical Center HCC registry has been described in our previous paper [10]. Briefly, all newly-diagnosed HCC patients who received care at our institution were prospectively registered.…”
