Study Objective: To assess, based on 3D-ultrasonography data, endometrial receptivity to implantation in women who have undergone conservative surgery for ovarian endometriomas (OE). Study Design: This was a prospective, comparative, clinical study. Materials and Methods: One hundred and seventy-two women who had undergone excision of OE were examined in the study: 114 patients with anti-Müllerian hormone (AMH) levels below 1.2 ng/mL (Group I, including subgroup A made up of 44 women younger than 35 and subgroup B made up of 70 women aged 35 to 40) and 58 patients with AMH ≥1.2 ng/mL (Group II). 3D-ultrasonography was performed at six and 12 months after cyst removal (between days 6 and 8 after ovulation). The VOCAL software was used to assess endometrial volume and volumetric blood flow indices, such as vascularization index (VI), flow index (FI), and vascularizationflow index (VFI). Study Results: One year after the surgery, endometrial volume in women from subgroup IA and Group II and volumetric blood flow indices in women from Group II did not significantly differ from population mean values. In subgroup IB endometrial gland volume remained reduced throughout the observation period; hemodynamics did not improve in either the endometrium (VI: 2.39 ± 1.57% after six months and 2.32 ± 1.44% after 12 months) or the subendometrial layer (VI: 10.96 ± 5.68% and 11.86 ± 4.59%, respectively). Conclusion: The structural and hemodynamic abnormalities revealed in the endometrium in the postoperative period should be viewed as factors worsening endometrial receptivity to implantation. Background. Currently, there is an increase in benign ovarian tumors, which, according to the results of numerous studies, negatively affect the reproductive health of women. Patients seeking to preserve their reproductive function prefer organ-preserving surgical interventions, however, in clinical practice, adnexectomy is often necessary due to the absence of intact ovarian tissue. Aim. Analysis of the morphofunctional state of the endometrium and its receptivity in patients with serous and mucinous ovarian cystadenomas before and after surgical treatment. Methods. The prospective study included patients of reproductive age with benign epithelial tumors: serous/mucinous cystadenomas who had not received hormonal drugs for six months prior to surgical treatment. 32 patients with serous cystadenoma were included in group I, 30 patients with mucinous cystadenoma in group II. Before and 6-12 months after unilateral salpingo-oophorectomy, a pipel biopsy of the endometrium was performed during the "implantation window". The qualitative and quantitative parameters of pinopodia were studied, the expression of estrogen (ER) and progesterone (PR) receptors in the glands and stroma of the endometrium were studied. Results. In the postoperative period, an increase in the number of mature pinopodia, a growing in the expression of ER in the stroma and PR in the stroma and glands was revealed. When analyzing the data in patients with mucinous cystadenoma After surgical treatment, in patients with mucinous cystadenoma, in comparison with the data of the serous patients in the glandular component of the endometrium, the expression of both ER and PR was lower, and the expression of PR in the stromal component was also lower, a growing in the number of developing pinopodia was recorded. Conclusion. The results of our study showed that benign ovarian tumors is accompanied by disorders of the morphofunctional state and receptivity of the endometrium.

Аbstract. Relevance. The number of women of older reproductive age is steadily increasing, and repeated failures of Assisted Reproductive Technologies programs during the transfer of high-quality embryos indicate the possibility of disruption of embryo implantation processes associated with impaired receptivity and functionality of the endometrium. Morphological, immunological and immunohistochemical changes in the endometrium associated with age factor may be decisive for the formation of the «implantation window» and correction of these changes and may improve the outcomes of Assisted Reproductive Technologies for a cohort of patients of older reproductive age. The aim of the study - to expand the pathogenetic understanding of the violation of the implantation ability of the endometrium in healthy patients of older reproductive age. Materials and Methods. A prospective sample study of 46 patients (group 1), aged 38 to 45 years with an officially registered diagnosis of infertility lasting no more than 4 years, with a successful gynecological and obstetric history, who were about to have their first IVF attempt, was conducted. The patients were examined according to Order № 803n of the Ministry of Health of the Russian Federation. Additionally, the level of peripheral blood melatonin, the determination of progesterone, estrogen, HLA-DR (MHC II), CD56 (NK cells), CD138, leukemia inhibiting factor receptors in the endometrium were studied. Concentrations of IL-6, IL-10, TGFß, and VGEF were determined in the cervical secretion, with the calculation of the pro-inflammatory index, as the ratio of IL-6/IL-10 cu and the ratio of TGFß1/VEGF. Statistical data processing was performed using the Statistica 10.0 application software package (StatSoft, Inc., USA). Results and Discussion. In the group of healthy patients of older reproductive age, there is an imbalance of steroid receptors and secretory transformation of the endometrium against the background of relative hyperestrogenism, with a decrease in the reception of own hormones in the endometrium. A decrease in melatonin signals a disorder of pineal and pituitary control over ovarian cycling. There is a decrease in the expression of leukemia inhibiting factor. Signs of inactive chronic endometritis with an autoimmune component are monitored, confirmed by a pro-inflammatory cytokine balance. The predominance of fibrosis processes over angiogenesis processes is confirmed by an increase in the ratio of TGFß1/VEGF and highly resistant blood flow in the uterine arteries. Conclusion. Standard pre-gravidar preparation cannot compensate for all factors that violate the implantation potential of the endometrium in this cohort of patients and requires the development of new complex techniques that directly affect the diversity of all factors that ensure the natural extinction of reproductive potential in order to increase the effectiveness of Assisted Reproductive Technologies programs.
