Medical treatment of hypertension was undertaken in Rochester, Minn. Mayo Clinic and Mayo Foundation: Section of MedicineDuring the past 10 years particular attention has been paid to surgical correction of occlusive lesions of the renal arteries in patients with diastolic hypertension. Methods of detecting these lesions and assessing their functional significance have been clearly established and have contributed materially to very favorable results following renovascular surgery on carefully selected patients. Understandably, much effort has been directed to more reliable selection of such patients for surgical intervention. However, little attention has been paid to the subsequent course of those hypertensive patients with renovascular disease who, for one reason or another, do not undergo surgical treatment. ZusammenfassungEs wird auf die Tatsache hingewiesen, daß das erste Teilstück der nephrographischen Kurve nicht als Ausdruck der Durchblutungsgröße der Nieren gewertet werden kann, da sich bei Nierenarterienstenose häufig ein erhöhtes I. Segment findet.Die verminderte Durchblutung der Niere ist dagegen besser durch einen flachen Verlauf des II. Segmentes zu erkennen.Die nephrographischen Befunde bei Nierenarterienstenose zeigen ein sehr vielfältiges Bild, wobei ein erhöhter, vor allen Dingen aber ein verspäteter Peak noch am häufigsten als charakteristisches Zeichen für das Vorliegen einer Minderdurchblutung gewertet werden kann. Die Treffsicherheit der Nierenuntersuchungen mit radioaktiven Substanzen kann bei Nierenarterienstenose durch eine scintigraphische Untersuchung ganz wesentlich verbessert werden.Bei Nierenarterienstenose findet sich im Scintigramm in der Regel eine vermehrte Anreicherung von Radio-Salyrgan* im Nierenbecken. Dieses Zeichen kann dann als pathognomonisch gewertet werden, wenn röntgenologisch ein erweitertes Nierenbecken ausgeschlossen wurde. Weiterhin wird auf die methodischen Vorteile hingewiesen, die nephrographische Untersuchung ohne Lageveränderung des Patienten im Anschluß an die Nierenscintigraphie durchzuführen. Bei diesem Vorgehen ist eine exakte Einstellung der Meßköpfe über dem Nierenmittelpunkt gewährleistet. Die Gammastrahlung des Hg203-Salyrgair* wird dabei durch eine geeignete Einstellung der Impulshöhenanalysatoren ausgeschaltet, so daß die Nephrographie mit J131-Hippuran* ohne Schwierigkeiten durchgeführt werden kann.
