The Pulmonary Haemodynamic Effects of Almitrine Infusion in Men with Chronic Hypercapnia

Abstract: 1. Almitrine, an agonist of peripheral chemoreceptors, has been shown to be effective in lowering PaCO2 in patients with chronic obstructive lung disease. The aim of this investigation was to evaluate the pulmonary haemodynamic response to almitrine in clinically stable patients with chronic airflow obstruction and chronic hypercapnia (PaCO2 7.1 +/- 0.5 kPa, mean +/- SD). 2. Seven men, aged from 55 to 64 years, had the following values for pulmonary function (means +/- SD): FEV1.0 0.67 +/- 0.16 litre; VC 2.12 … Show more

“…One small trial, however, did show rises 1131. Tests with almitrine given acutely led only to transient increases in Ppa [14]. In this connection, the dilator effect, observed by several groups as well as ourselves, must be born in mind [2-41; this might counteract any tendency to a rise in Ppa caused by its vasoconstrictor action.…”

“…An acute rise of PAP and pulmonary vascular resistance has been observed after intravenous infusion of almitrine [5][6][7] and was generally attributed to an enhancement of hypoxic vasoconstriction by the drug. When almitrine was given orally the short-term results were contradictory and varied from no increase in PAP [11] to a slight but significant increase [8] and to a marked increase [9).…”

“…However, there is still some uncertainty about the pulmonary haemodynamic effects of almitrine, since an acute increase of pulmonary artery mean pressure (PAP) has been observed in short-term studies after either intravenous infusion of almitrine [5][6][7] or oral intake of the drug [8,9), but these results were not confirmed by other studies [10,11). One to six month's treatment with oral almitrine has been shown to increase PAP (9) or was not associated with any change in pulmonary haemodynamics [12][13][14].…”

“…It would be difficult to detect, because tests cannot be made under the controlled conditions available in animals. Nevertheless the rise in PPA which it has caused in COAD is often ill-sustained despite continued intravenous infusion (Dull et al 1983).…”
