The Prognostic Superiority of Log Odds of Positive Lymph Nodes in Stage III Colon Cancer

Abstract: Colon cancer patients with LNR4 disease represent a heterogeneous group. The previously reported prognostic association of TNODS and LNR and outcome of stage III disease were confounded by LODDS.

“…The results highlight the prognostic World J Surg (2012) 36:1906-1914 1911 role and value of lymph node evaluation in four perspectives, described in the paragraphs that follow. First, through a correlation study among these five types of evaluation, we noted that LNN highly correlated with LNT, and that LODDS highly correlated with LNR, findings consistent with a prior report from Wang et al [17]. In that study, they observed a marked association between LNT and LNN.…”

“…Also, some investigators have reported that various types of lymph node evaluation besides the total number of lymph nodes retrieved (LNT) can effectively predict long-term outcome, including number of negative lymph nodes (LNN) [4], number of positive lymph nodes (LNP) [13][14][15], ratio of positive lymph nodes (LNR) [2,11,12,16], and log odds of positive lymph nodes (LODDS) [17]. In the meantime, investigators discovered cut-off values other than the benchmark number that could have good prognostic value [18][19][20].…”

“…LODDS, a novel indicator of predicting lymph node status, may provide a new means for improving the accuracy of N classification for prognostic assessment. Previous studies have indicated the superiority of LODDS over LNR in terms of prognostic significance in colon cancer (17,23,24). However, the available data remain limited.…”

“…The log odds of positive lymph nodes (LODDS), another novel prognostic indicator, was recently introduced (17) and it is defined as the log of the ratio between the number of positive and the number of negative lymph nodes. The prognostic significance of LODDS in gastric, pancreatic and breast cancer was previously investigated (18)(19)(20)(21)(22) and certain studies have indicated the superiority of LODDS over LNR in terms of prognostic significance in Log odds of positive lymph nodes is superior to the numberand ratio-based lymph node classification systems for colorectal cancer patients undergoing curative (R0) resection (17,23,24). The value of LODDS remains unclear, and studies comparing the prognostic value among the pN, LNR and LODDS classifications for CRC following R0 resection have been sparse.…”

“…The log odds of positive lymph nodes (LODDS) have been regarded a novel prognostic indicator in many other cancers, such as pancreatic cancer, gastric cancer, and colorectal cancer, defined as the log of the ratio between the probability of being a positive LN and the probability of being a negative LN when one LN is dissected (9)(10)(11)(12)(13)(14). However, in dealing with advanced ESCC patients (pN0 stage especially) after curative surgery, rare attention has been paid to the prognostic value of LODDS.…”
