The prognostic role of preoperative serum C-reactive protein in predicting the biochemical recurrence in patients treated with radical prostatectomy

Abstract: Preoperative CRP is elevated in patients with pathological features of aggressive PCa and BCR after RP. While CRP has independent prognostic value, it does not add prognostically or clinically significant information to standard predictors of outcomes.

“…In particular, we focused attention to the levels of BIRC5, PSA, SRD5A2, MMP2 CRP and SOD2 genes, known to be involved in PCa initiation and progression [3841], and associated with inflammatory [42] and oxidative stress conditions. The results showed an increase of the MMP2 , BIRC5, CRP and SOD2 expression in samples characterized by high Gleason Score.…”

“…Similarly, patients with elevated preoperative CRP were more likely to have extracapsular extension, seminal vesicle invasion, lymph node metastasis, positive surgical margins, and biochemical recurrence (23). Although elevated postoperative CRP was associated with adverse pathologic outcomes and higher rates of 5-year biochemical recurrence, the addition of CRP to established pre-operative modeling for BCR did not improve predictive modeling over the traditional characteristics alone (23,52). As such, CRP likely has limited utility for outcome prediction after RP.…”

“…Limited data exists regarding CRP in the setting of localized prostate cancer treated with radical prostatectomy (RP). In a retrospective, multi-institutional study of 7,426 patients who underwent RP, pre-prostatectomy CRP levels were associated with pathologic Gleason score, but not tumor stage (22,23). Similarly, patients with elevated preoperative CRP were more likely to have extracapsular extension, seminal vesicle invasion, lymph node metastasis, positive surgical margins, and biochemical recurrence (23).…”

“…C-reactive protein (CRP), an acute phase reactant that is elevated in IBD, was also associated with higher PSA levels [32][33][34]. In fact, higher CRP levels were found to correlate with PCa resistance to treatment and poorer survival [34,35].…”
