The prognostic importance of scalp location in primary head and neck melanoma

Abstract: Background and Objectives For patients with cutaneous melanoma, primary tumors located in the head and neck is associated with poor outcomes. The reason for this difference and whether it is applicable to all locations within the head and neck remains unclear. We hypothesized that scalp melanoma is uniquely distinguished from other anatomic sites and is independently responsible for the poor prognosis of head and neck melanoma. Methods Query and analysis of a prospectively maintained melanoma database of all… Show more

“…Thereby, we found not only that thin melanomas in the head and neck region (scalp excluded) were associated with death, but we were also able to identify the scalp as an even stronger risk factor. Fatal thin melanomas were sixfold as likely to arise on the scalp than nonfatal thin melanomas, thus strengthening earlier suggestions that this anatomical location is an important predictor . Clinically, scalp melanomas may be concealed in hair‐bearing areas.…”

“…Both of these factors seem to be the key contributors responsible for the greater primary thickness of scalp melanomas. 3,7,8,20 6 | MORPHOLOGIC CLASSIFICATION OF SCALP MELANOMAS 6. Acral lentiginous melanomas are typically found on the palms, soles and subungual locales of the digits or toes.…”

“…[1][2][3][4][5][6][7] 2 | SCALP MELANOMAS: MAGNITUDE OF THE PROBLEM Scalp melanomas account for 7% of all melanomas. 8 Certain benchmark publications have reported scalp melanomas either as a combined category of "scalp and neck" melanomas or as a larger group of "head and neck" melanomas. Therefore, from these studies we can comprehend only the relative frequency of scalp melanomas within the head and neck region.…”

“…It is known that melanomas located in different locations may have different behavior. Specifically head and neck melanoma have been shown to have poor survival and truncal melanomas are more likely to have positive sentinel nodes ( 15 , 18 ). This is particularly important to this study as melanomas in these areas are the most likely to have MDB.…”
