The Prevention of Coronary Artery Aneurysm in Kawasaki Disease: A Meta-analysis on the Efficacy of Aspirin and Immunoglobulin Treatment

Abstract: Objective. Varying observations have been made concerning the use of aspirin (ASA) and/or intravenous immunoglobulin (IVIG) in the prevention of coronary artery aneurysm (CAA) in children with Kawasaki disease. A meta-analysis of published articles on the subject was conducted to evaluate the reported efficacy of these therapies. Methods. All published studies in all languages from 1967 through 1993 obtained from MEDLINE and EMBASE were considered, and a defined set of inclusion and exclusion cr… Show more

“…At present, the treatment of choice for acute KS is a single dose of IVIG, at 2 gm/kg administered over 10-12 hours, in combination with ASA (62). Compared with multiple-dose regimens of approximately equivalent total dose, this single high dose of IVIG has been associated with a lower incidence of coronary abnormalities, more rapid resolution of fever and laboratory indices of acute inflammation, reduced duration of hospitalization, and higher peak serum IgG levels (7).…”

“…Most infants and young children with KD received oral aspirin and IVIG therapy. [20][21][22][23][24][25][26] Corticosteroid therapy or infliximab as biological therapy is also used and effective in the treatment of IVIG-resistant KD. 24,[27][28][29] Two patients were treated only with aspirin, and 17 patients received a combination of oral aspirin and IVIG as shown in Table 1.…”

“…[4,5] The mainstay of treatment is IVIG and aspirin, which have been shown to reduce the incidence of coronary artery aneurysms. [6] Unfortunately, not all children respond to IVIG, which poses a management dilemma. [7][8][9][10] Multiple large scale epidemiological studies have been geared towards better understanding the etiology and improving therapeutic interventions.…”
