Sports trauma frequently involves insult to oral soft and hard tissues resulting in loss of tooth structure and tooth loss. Multiple sporting equipment's are employed to prevent irreversible damage to oro-facial structures, and to reduce the overall cost of treatment. However in the undesired consequence of loss of oral structures, multiple treatment options are utilized to restore, esthetics, comfort and function. Osseointegrated implant supported rehabilitation of oral structures in trauma cases provides highly predictable treatment outcomes along with preservation of remaining tissues. This case report presents the management of avulsed teeth caused by hockey stick injury, with osseointegrated dental implant supported fixed partial dentures in esthetic zone using contemporary restorative techniques.

Chronic traumatic encephalopathy (CTE) is a progressive neurodegenerative disorder linked to repetitive traumatic brain injury (TBI) and characterized by deposition of hyperphosphorylated tau at the depths of sulci. We sought to determine the presence of chronic traumatic encephalopathy (CTE) pathology in a brain bank for neurodegenerative disorders for individuals with and without a history of contact sports participation. Available medical records of 1,721 men were reviewed for evidence of past history of injury or participation in contact sports. Subsequently, cerebral cortical samples were processed for tau immunohistochemistry in cases with a documented history of sports exposure as well as age- and disease-matched men and women without such exposure. For cases with available frozen tissue, genetic analysis was performed for variants in APOE, MAPT, and TMEM106B. Immunohistochemistry revealed 21 of 66 former athletes had cortical tau pathology consistent with CTE. CTE pathology was not detected in 198 individuals without exposure to contact sports, including 33 individuals with documented single-incident TBI sustained from falls, motor vehicle accidents, domestic violence, or assaults. Among those exposed to contact sports, those with CTE pathology did not differ from those without CTE pathology with respect to noted clinicopathologic features. There were no significant differences in genetic variants for those with CTE pathology, but we observed a slight increase in MAPT H1 haplotype, and there tended to be fewer homozygous carriers of the protective TMEM106B rs3173615 minor allele in those with sports exposure and CTE pathology compared to those without CTE pathology. In conclusion, this study has identified a small, yet significant, subset of individuals with neurodegenerative disorders and concomitant CTE pathology. CTE pathology was only detected in individuals with documented participation in contact sports. Exposure to contact sports was the greatest risk factor for CTE pathology. Future studies addressing clinical correlates of CTE pathology are needed.

Résumé -Introduction :La commotion cérébrale dans le sport ou Sport-Related Concussion (SRC) est un processus complexe se traduisant par un trouble de fonctionnement du cerveau. Son incidence ne cesse d'augmenter car la mise en place de mesures efficaces de prévention achoppe sur les zones d'incertitude entourant les SRC (diagnostic, traitements, etc.). Revue de littérature : Dans les années 1960, certains auteurs ont avancé l'hypothèse que le port d'un protège-dents ou protection intrabuccale (PIB) permettrait de prévenir la survenue de SRC. Depuis, des études confirmant ou infirmant cette hypothèse sont régulièrement publiées. Une analyse de la littérature permet de comprendre cette controverse et de distinguer ce qui est avéré de ce qui est supposé. Conclusion : Actuellement, sur la base d'arguments scientifiques, le port de PIB ne peut et ne doit pas être recommandé dans la prévention des SRC. La surestimation des effets protecteurs de la PIB risquerait d'engendrer une augmentation des comportements à risque chez les sportifs qui se sentiraient protégés alors qu'ils ne le seraient pas et ainsi d'augmenter paradoxalement la survenue de SRC. En revanche, l'efficacité des PIB dans la prévention des lésions oro-faciales étant scientifiquement prouvée, c'est à cette fin qu'elles doivent être recommandées pour la pratique sportive.Abstract -Investigation of the use of a mouthguard for decreasing the incidence and/or severity of sportrelated concussions. Introduction: Concussion in sport, so-called sport-related concussion (SRC), is a complex pathophysiological process resulting in the impairment of the brain. Its incidence is increasing due to the uncertainty surrounding SRC (diagnosis, management, etc.). Literature review: In the 1960's, some authors put forth the hypothesis that the use of a mouthguard could prevent the occurrence of SRC. Since then, many studies confirming or invalidating this hypothesis have been published regularly. An analysis of the literature allows us to understand this situation and distinguish between what is proven and what is supposed. Conclusion: Currently, on the basis of the scientific evidence, the use of a mouthguard cannot and must not be recommended for the prevention of SRC. The overestimation of the protective effects of a mouthguard can lead to an increase in risk behaviours in sportspeople, who feel protected while they are not. This can result in a paradoxical increase in SRC. However, mouthguards have been shown to be effective in preventing dental and orofacial injuries. For this reason, mouthguards should continue to be recommended and used in sport activities.
