Background: Policymakers are interested in investigating effects of governments' policies on socioeconomic inequality in public health. Objectives: This study aimed to analyze levels of and changes in socioeconomic inequality of unintended pregnancy after the changes in family planning policies and to investigate determinants of its changes in Iran. Methods: Required data were extracted from Iran's Multiple Indicator Demographic and Health Surveys conducted in 2010 and 2015. We used data from 1123 and 900 married pregnant women aged 15 -49 years in 2010 and 2015, respectively. Wagstaff normalized concentration index was used to measure unintended pregnancy inequality. The contribution of various factors to the measured inequality in 2010 and 2015 was investigated by decomposing concentration index. Changes in the unintended pregnancy inequality in 2010 -2015 and its determinants were assessed using Blinder-Oaxaca decomposition method. Results: Pro-rich unintended pregnancy inequality declined by 120% from -0.145 to 0.030 in 2010 -2015. However, the pro-poor unintended pregnancy inequality in 2015 was not statistically significant. Households' economic status and women's age at pregnancy were the two leading factors with positive contributions while contraceptive non-use before pregnancy and women's education level had the most negative contributions to the reduced pro-rich inequality of unintended pregnancy in 2010 -2015. Conclusions: Pro-rich unintended pregnancy inequality not only did not increase, but also declined to zero after the changes in family planning policies. Providing sustainable livelihood for disadvantaged households with women at reproductive ages can maintain this favorable condition in the future.

Background Unplanned pregnancy is a significant public health issue in both low- and high-income countries. The burden of unplanned pregnancy is reflected in women opting for pregnancy terminations and it can be detrimental to the women and her family as well as the health system and society. Solid data on the proportion of unplanned pregnancies are using more specific tools such as the London Measure of Unplanned Pregnancy (LMUP) needed to address the issue in Sri Lankan contexts. The objective was to describe the proportion of unplanned pregnancies, their determinants and the health outcomes of women delivering at Colombo North Teaching Hospital-Ragama (CNTH).Methods A cross-sectional study was carried out among 494 consecutive pregnant women selected by non-probability consecutive sampling who were admitted for the confinement at CNTH. A pre-tested structured interviewer-administered questionnaire was used to collect data on antenatal women and intentionality measured by self-administered six-item LMUP. Maternal and newborn health outcomes were ascertained in each post-partum women before discharge. Data were analyzed with the Mann-Whitney U tests, Kruskal-Wallis tests and spearman rank correlation. We also evaluated the psychometric properties of the Sinhalese version of LMUP. Results The response rate was 97.8% and 17.2% of pregnancies ending at birth were unplanned, 12.7% were ambivalent and 70.1% were planned. Associated factor profile of women with unplanned pregnancies includes; not married women (p=0.001), educated up to the passing of GCE ordinary level by women (p<0.001) and spouse (p<0.001), primiparity (p=0.002) and inadequate knowledge on emergency contraceptives (p=0.037). Less planned pregnancies were also significantly associated with anemia (p=0.004), low mood for last two weeks (p<0.001), having a partner with problematic alcohol consumption (p<0.001), presence of Gender-Based Violence (GBV) (p<0.001), poor relationship satisfaction with partner (p<0.001) and family (p<0.001). Inadequate pre-pregnancy preparation and antenatal care were associated with an unplanned pregnancy. No differences were found in neonatal outcomes. Sinhalese version of the LMUP scale was found to be accepted, valid and reliable with the Cronbach's alpha of 0.936. Conclusions A sizeable proportion of pregnancies were unplanned. Teenage pregnancies, non-marital relationships and inadequate knowledge on emergency contraceptives, maternal anemia, low mood, and GBV were modifiable associated factors which could be prevented by evidence-based locally applicable approaches.

Мета дослідження: оцінити клінікопрогностичну роль гормонального профілю фетоплацентарного комплексу при багатоплідній вагітності залежно від типу плацентації. Для проведення дослідження відібрано 114 пацієнток із діагностованою багатоплідною вагітністю. Всі спостереження проводилися на базі пологового відділення №1 КУ «Запорізький обласний перинатальний центр» ЗОР у період з 2013 по 2015 рр. Діагноз багатоплідної вагітності встановлювали за допомогою об'єктивного дослідження та підтверджували ультразвуковим дослідженням. Усіх пацієнток із багатоплідною вагітністю розподілили на дві групи залежно від хоріальності: І група -49 (42,98 %) пацієнток із монохоріальною двійнею, яка включала моноамніатичну ( 6 жінок) та діамніотичну (МД, 43 особи); ІІ група -65 (57,02 %) пацієнток із дихоріальною діамніотичною двійнею (ДД), адже саме хоріальність та амніотичність визначає перебіг вагітності, її наслідки, перинатальну захворюваність і смертність. Однако в нашем исследовании монохориальный тип плацентации характеризуется достаточно высоким уровнем компенсаторных возможностей гормоносинтетической функции фетоплацентарного комплекса и отсутствием существенных отклонений в уровнях гормонов в крови беременных женщин.Исследование гормонального профиля фетоплацентарного комплекса беременных с многоплодной беременностью подтвердило наличие хронической плацентарной дисфункции. Каждая 3-я беременная из обеих групп наблюдения имела низкие уровни неконъюгированного эстриола, каждая 4-ая -прогестерона и каждая 5-ая -плацентарного лактогена.Ключевые слова: монохориальная диамниотическая беременность, дихориальная диамниотическая беременность, фетоплацентарный комплекс, прогестерон, кортизол, эстриол, плацентарный лактоген.Abstract. Objective: to evaluate the clinical and prognostic role of placenta hormonal profile in multiple pregnancies, depending on the type of placentation.The study involved 114 patients diagnosed with multiple pregnancy. Diagnosis of multiple pregnancy was established by using an objective study and confirmed by ultrasound. All patients with multiple pregnancy were divided into 2 groups based on chorionic type: I group -49 (42,98 %) patients with monochorionic twins, which included monoamniotic (6 women) and diamniotic (43 people); II group -65 (57,02 %) patients with dichorionic diamniotic twins, because chorionic and amniotic type determines pregnancy development, perinatal morbidity and mortality.Analyzing our results of hormonal function of the placenta in multiple pregnancies we indicated that such type of placentation as dichorionic diamniotic is more favorable for pregnancy and shows fewer complications in this pregnancy compared to monochorionic diamniotic type of placentation.
