ENGLISH SUMMARYOtitis media refers to an infection or inflammation of the middle ear cavity that also affects the eustachian tube and the mastoid cells. Otitis media is almost always caused by an ascending pathology of the epithelial and subepithelial layers of the middle ear mucoperiostium. This paper firstly describes the classification and distribution of otitis media. Next, using Norwegian data, host related risk factors for otitis media are discussed, particularly focusing on gender differences.I denne artikkelen beskrives inndeling og forekomst av sykdommen, som er hyppig i barneårene og avtar med økende alder. Det er en betydelig genetisk disposisjon for mellomøresykdom. Med utgangspunkt i norske data vil vertsrelaterte og miljørelaterte risikofaktorer for sykdommen bli presentert. Spesielt er vertsforhold og kjønnsulikheter vektlagt i diskusjonen.
INNDELING AV MELLOMØREBETENNELSEROtitis media er definert som en inflammasjon eller betennelsestilstand av mellomøret. Sekretorisk otitt etterfølger vanligvis akutt otitt. Kronisk otitt med påfølgende skade på mellomørebena og trommehinnen kan etterfølge gjentatte akutte otitter og langvarig sekretorisk otitt, men sees også uten tidligere symptomer fra ørene (1). Paparella og medarbeidere har vist i eksperimentelle studier at de histopatologiske forandringene i mellomøret som følger de ulike sykdommene er manifestasjoner av samme sykdom, i ulike faser (1).Akutt otitis media er definert som en klinisk identifiserbar mellomøreinfeksjon, akutt innsettende og av kort varighet. Akutt otitt er vanlig i barneårene, omlag 3/4 av alle barn har hatt minst en ørebetennelse før fem års alder (2). Enkeltepisoder er forbigående og oftest ukompliserte, mens barn med stadig tilbakevendende akutte otitter, «ørebarna», representerer en tidkrevende og behandlingsmessig komplisert gruppe. «Ørebarna» representerer en liten gruppe barn som debuterer tidlig med stadig tilbakevendende akutte øreinfeksjoner (3,4). I forskningssammenheng er begrepet «ørebarn» brukt om barn som har tre eller flere otitter i løpet av et halvt år eller fire eller flere otitter i løpet av ett år (5,6). Barna er lett karakteriserbare i den kliniske hverdag. De har stadige tilbakevendende øre-infeksjoner, og en er ofte i tvil om hvorvidt det har vaert en infeksjonsfri periode mellom to episoder.
