The prevalence of exclusive breastfeeding (EBF) is suboptimal in Nepal and very low in Madhesh province. Dalits are generally known for being economically exploited, politically voiceless socially degraded educationally backward classified as untouchables and deprived of human dignity and social justice. This study aimed to assess the proportion of Dalit Mothers practicing exclusive breastfeeding and the associated factors. The study utilized an analytical crosssectional design by using a semistructured questionnaire to 156 Dalit mothers of Rajbiraj Municipality Saptari Nepal. For the analysis involving testing for the association of independent variables with dependent variables bivariate analysis (Chisquare statistics and a p value at <0.05) was used. Those significance in the bivariate analysis were further preceded by multivariate logistic regression. In the present study the prevalence of exclusive breastfeeding was (43.6%). Mothers occupation (AOR = 5.724; CI = 1.726 to 18 977) Mothers education (adjusted odds ratio [AOR] = 5.724; CI = 1.726 to 18 977) smoking habit (AOR = 2.785; CI = 1.073 to 7.228) colostrum milk feeding (AOR = 14.199; CI = 3.508 to 57.474) number of times visit the health center for ANC (AOR = 2.852; CI = 1.210 to 6.726) were positively associated with exclusive breastfeeding whereas sex of the child type of family knowledge about breastfeeding mothers receive counseling on EBF in ANC or PNC visit was also associated with exclusive breastfeeding. Among the respondents who did not practice exclusive breastfeeding the common reasons cited were trouble initiating milk flow (23.7%) insufficient breast milk production (21.7%) Breast milk not satisfying the baby (20.9%) too many household duties (12%) difficulties with infant sucking or latching (6.4%) and the baby becoming sick and not being able to breastfeed (4.8%). The study demonstrates that nearly half (43.6%) of the infants were breastfed exclusively hence the need for targeted interventions and support to promote EBF among Dalit mothers. Factors such as maternal education occupation colostrum milk feeding and antenatal care visits were identified as essential influencers of exclusive breastfeeding. Healthcare professionals policymakers and community leaders can utilize these insights to design effective strategies that encourage and support exclusive breastfeeding among Dalit mothers ultimately improving the health and wellbeing of infants in this marginalized community.

BackgroundExclusive breastfeeding (EBF) is a major public health problem in Ethiopia. However, the spatial variation of EBF and the associated factors have not been studied as much as we have searched. This study aimed at assessing geospatial variation and the predictors of EBF using geographically weighted regression.MethodsA cross-sectional study was conducted using the 2019 Mini-Ethiopian Demographic and Health Survey data set. The study used a total weighted sample of 548 infants. Hotspot spatial analysis showed the hotspot and cold spot areas of EBF. The spatial distribution of EBF was interpolated for the target population using spatial interpolation analysis. SaTScan V.9.6 software was used to detect significant clusters. Ordinary least squares regression analysis identified significant spatial predictors. In geographically weighted regression analysis, the effect of predictor variables on the spatial variation of EBF was detected using local coefficients.ResultsThe weighted prevalence of EBF in Ethiopia was 58.97% (95% CI 52.67% to 64.99%), and its spatial distribution was found to be clustered (global Moran’s I=0.56, p<0.001). Significant hotspot areas were located in Amhara, Tigray, Southern Nations, Nationalities, and Peoples’ Region, and Somali regions, while significant cold spots were located in Dire Dawa, Addis Ababa and Oromia regions. Kulldorff’s SaTScan V.9.6 was used to detect significant clusters of EBF using a 50% maximum cluster size per population. The geographically weighted regression model explained 35.75% of the spatial variation in EBF. The proportions of households with middle wealth index and married women were significant spatial predictors of EBF.ConclusionMiddle wealth index and married women were significant spatial predictors of EBF. Our detailed map of EBF hotspot areas will help policymakers and health programmers encourage the practice of EBF in hotspot areas and set national and regional programmes focused on improving EBF in cold spots by considering significant predictor variables.

Background The benefits of exclusive breastfeeding (EBF) practices have been universally documented, with evidence of one positive impact on a child’s optimal growth, development, and survival. However, EBF practice in Nepal has fluctuated and declined in the last 25 years. Along with the individual factors, it is also important to acknowledge that the EBF practices are affected by multiple community-level factors. Understanding these factors is essential to designing breastfeeding promotion programs to improve child health outcomes in Nepal. This study investigated individual and community-level determinants of EBF practices among infants aged 0-5 months in Nepal. Method We used the dataset from the Nepal Demographic and Health Survey 2022. Information on exclusive breastfeeding in the past 24- hours was available for 540 infants aged 0-5 months. A multilevel mixed-effect logistic regression was used to identify individual and community-level factors associated with EBF practices among infants aged 0-5 months in Nepal. Result The 24-hour prevalence of EBF among infants aged 0-5 months was 57.46% (95% Confidence Interval (CI): 52.18, 62.57). The prevalence decreased with age; it was 81.2% at one month and 40.6% at five months. Several individual and community-level factors appeared significant. The infant’s age was inversely associated with EBF prevalence at the individual level. In multilevel logistic regression, infants aged three months (Adjusted Odds Ratio (AOR): 0.14, 95% CI: 0.05, 0.40), four months (AOR: 0.11, 95% CI: 0.04, 0.28), and five months -(AOR: 0.07, 95% CI: 0.03, 0.20) had a lower likelihood of receiving EBF relative to infants less than one-month-old. At the community level, mothers from communities with moderate ANC ≥ 4 coverage (AOR: 3.30, 95% CI: 1.65, 6.57) and high ANC ≥ 4 coverage (AOR: 2.70, 95% CI: 1.40, 5.22) had higher odds of EBF compared to communities with low coverage. Similarly, communities with moderate levels of maternal employment (AOR: 2.67, 95% CI: 1.34, 5.30), high levels of maternal employment (AOR: 2.34, 95% CI: 1.10, 4.99), and moderate levels of poverty (AOR: 2.20, 95% CI: 1.13, 4.28) were positively associated with EBF. Regional variation was evident, with infants in Lumbini province having lower odds of EBF (AOR:0.32, 95% CI: 0.13, 0.77) compared to Koshi province, whereas those in Sudurpaschim having higher odds (AOR: 2.20, 95% CI: 1.13, 4.28). Around 9% Variation in EBF was observed while mapping across clusters in this study. Conclusion Our study demonstrated various individual and community-level factors impacting EBF practices in Nepal, underscoring the need for improving EBF programs. It highlighted unique community-level factors such as ANC coverage, poverty, and maternal employment as the factors influencing EBF practices, and it showed around 9% variability in EBF practices. Future breastfeeding promotion efforts should focus on older infants and communities with low poverty levels and low coverage of recommended ANC ≥ 4 visits. Furthermore, context-specific adaptation of such efforts might be required as there was variation between the communities.
