Assessment of Hematological Parameters in Pulmonary Tuberculosis Patients

The aim of the study was the assessment of hematological parameters in pulmonary tuberculosis patients. Forty patients diagnosed with tuberculosis were recruited from the Institute of Thoracic Medicine on the basis of history, clinical examination, chest radiography, sputum examination and related laboratory parameters and were compared with age and sex matched healthy volunteers (n = 40). Hematological parameters and CRP in tuberculosis patients were determined. The mean values for serum hemoglobin level, RBC count and platelet count in PTB was found to be less (p \ 0.001). Erythrocyte sedimentation rate (ESR), plasma C-reactive protein, WBC count in PTB subjects was increased (p \ 0.001 for ESR & CRP, p \ 0.05 for WBCs) and all were statistically significant. This study demonstrated that serum hemoglobin level, RBC count and platelet count was decreased in tuberculosis patients whereas ESR, CRP and WBC count was increased when compared with healthy controls.

Context: The burden of TB in Africa tends to be exacerbated by the socio-economic situation and the high prevalence of intercurrent infections such as HIV, malaria and non-specific bacterial infections. These factors often result in anemia, making patients at high risk for anemia. Objective: We aimed to gain insights into the characteristics of anemia, hematologic variations and socio-economic status in untreated pulmonary TB patients (PTB) in Kinshasa, the Democratic Republic of Congo. Methods: We conducted a cross-sectional analysis of 200 smear-positive pulmonary TB patients (PTB) recruited at the initiation of TB treatment. Complete Blood Count, Iron profile, BMI, CRP and albuminemia were assessed. Data were analyzed using Student t or Mann Whitney tests as appropriate, and logistic regression was performed to assess the strength of associations. Results: Anemia was a regular finding in (69%). This anemia was mostly moderate (92.2%) and with iron deficiency pattern (48%). Hypoalbuminemia was observed in half of the subjects and appears to be correlated with the severity of anemia. Surprisingly, the severity of inflammation, as reflected by the CRP, was inversely correlated with the anemia. In the multivariate analysis, alcohol intake (OR: 2.38; IC 95%: 1.05 -5.38), hypoalbuminemia (OR: 1.98; IC 95%: 1.02 -3.82) and CRP rate were significantly associated with the presence of anemia among pulmonary tuberculosis at the diagnostic. Conclusion: This study demonstrates the heavy burden of the iron responsive anemia and risky life conditions in newly diagnosed TB patients, and underscores the potential usefulness of iron supplementation in the Congolese context.

Amaç: Bu çalışmada yayma ve kültür negatifliği saptanan ve sonrasında histopatolojik inceleme ile tanı konulmuş akciğer tüberkülozlu hastaların klinik ve radyolojik özellikleri araştı-rıldı.Ça lış mapla nı:Ocak 2007 -Ocak 2011 tarihleri arasında kliniğimizde tedavi edilen 354 akciğer tüberkülozu tanılı erkek hastanın (ort. yaş 28.3±9.4; dağılım 21-47 yıl) tıbbi kayıtları retrospektif olarak incelendi. Bu hastalardan 16'sının (%4.5) akciğer tüberkülozu tanısı histopatolojik olarak da kanıtlan-mış idi. Bul gu lar:En sık görülen semptomlar öksürük (%56), gece terlemesi (%25) ve göğüs ağrısı (%25) idi. On bir hastanın tüberkülin cilt testi (TCT) sonucuna ulaşıldı ve ortalama TCT ölçümü 15.1±2.7 mm (dağılım 12-20 mm) idi. Anormal laboratuvar bulgusu olarak sedimentasyon hızı yüksekliği saptandı. En sık görülen lezyonlar tüberkülom (%50), eksüda-tif lezyon (%43), fibroproliferatif lezyon (%12), konsolidasyon (%12), tomurcuklu ağaç işareti (%12) idi. Bir hastada kavite (%6) ve bir başka hastada bronşiektazi (%6) saptandı. Beş hastada birden fazla lobda lezyon yerleşimi saptanır iken, 11 hastada tek lob yerleşimi mevcuttu. Sağ akciğerde 16 lezyon, sol akciğerde sekiz lezyon saptandı. Sağ üst lob tutulumu dokuz hastada (%56), sağ orta lob tutulumu bir (%6), sağ alt lob tutulumu altı (%37) ve sol üst lob tutulumu dört (%25), sol alt lob tutulumu dört hastada (%25) görüldü.So nuç:Çalışma bulguları, histopatolojik inceleme gereken formların başında tüberkülomun geldiğini göstermektedir. Akciğer tüberkülozu klinik ve radyolojik belirtileri diğer hastalıklarla karışabilir; bu nedenle mikrobiyolojik olarak kesin tanı konulamayan hastalarda histopatolojik incelemeler tüberküloz tanısına ulaşmamızı sağlayabilir.Anah tar söz cük ler: Klinik özellikler; histapatolojik inceleme; akciğer tüberkülozu; radyolojik özellikler.
